心律平与三磷酸腺苷治疗小儿室上性阵发性心动过速82例对比观察

摘　　要：目的:探讨心律平(Propafenone)与三磷酸腺苷(ATP)治疗小儿室上性阵发性心动过速(PSVT)的疗效和不良反应。方法:将室上性阵发性心动过速(室上速)82例患儿分为心率平治疗组(38例)和三磷酸腺苷治疗组(44例),分别应用静脉注射心律平和三磷酸腺苷观察疗效和不良反应。结果:三磷酸腺苷对室上性心动过速的有效率为93.2%,心律平对室上性心动过速的有效率为78.9%,三磷酸腺苷组有效率明显高于心律平组(P<0.05);三磷酸腺苷对室上性心动过速的复律时间为21.2±7.8s,心率平对室上性心动过速的复律时间为18.4±7.5min,三磷酸腺苷组复律时间明显短于心率平组(P<0.05)。结论:心律平与三磷酸腺苷治疗小儿室上性阵发性心动过速均有效,但三磷酸腺苷有效率及复律时间明显优于心率平,不良反应虽大于心率平,但持续时间短且可自行缓解,应作为儿科首选。Objective：To study the clinical effects and side effects of Propafenone and ATP in treatment of children suffered from Paroxysmal Superventricular Tachycardia（PSVT）.Methods： Eighty-two children diagnosed with PSVT were divided into two groups,one group（n=38） treated with Propafenone,the other group（n=44） treated with ATP,all injected through peripheral venous and monitored for clinical effects and any side effects.Results： The effective rate of ATP in treatment of PAVT was 93.2%,whereas the effective rate of Propafenone was 78.9%,there was statistical difference between ATP group and propafenone group（P0.05）;time need to restore the sinus rhythm of the heart in ATP group was（21.2±7.8） seconds,and the value was（18.4±7.5） seconds in Propafenone group.It showed that ATP group was more effective than Propafenone group and there was statistical difference between these two groups（P0.05）.Conclusion：ATP and Propafenone were all effective in treatment of children suffered from PSVT,but the effective rate and the time need to restore the sinus rhythm of the heart in ATP group were better than Propafenone group.In regard of side effect,ATP was severe than Propafenone,but it only lasted for a short time and could remitted spontaneously.So ATP might be the first choice in treatment of PSVT in pediatrics.
