摘 要:目的探讨妊娠早中期妇女亚临床甲状腺功能异常对后代甲状腺功能、智力和身体发育情况的影响。方法选择2004年7月—2006年1月40例妊娠早中期亚临床甲状腺功能异常妇女及30例健康妇女的后代分别作为研究组和对照组。测定两组婴儿在新生儿期、12、24、36个月龄时的甲状腺功能[游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)]、发育商、语言和功能构成、身长及体质量等指标,比较两组婴儿有无差异。结果新生儿期研究组的TSH显著高于对照组,血清FT4、FT3水平明显低于对照组(P<0.05)。12、24、36个月两组的TSH、FT4、FT3水平差异均无统计学意义(P>0.05)。研究组的发育商在新生儿期、12、24、36个月均明显低于对照组(P<0.05)。研究组的身体发育在新生儿期、12、24、36个月均滞后于对照组(P<0.05)。结论妊娠早中期妇女亚临床甲状腺功能异常可能给后代的智力、身体发育造成不良影响,又可能引起短期的甲状腺功能异常,提示筛查和积极治疗妊娠早中期亚临床甲状腺异常对优生优育的必要性,加强患甲状腺疾病的母亲及其孩子的管理十分重要。Objective To investigate the influence of maternal subclinical thyroid abnormality during early and midtrimester pregnancy over the offspring′s thyroid function,intellectual development and body development.Methods A total of 40 children whose mothers were subclinical thyroid abnormality patients in Xinyi People′s Hospital between July 2004 to January 2006 were enrolled as the case group.Additional 30 children whose mothers were healthy were also collected as control group.All the 70 children′s thyroid function(FT3,FT4,TSH),intelligent quotient,language development,height and weight when children were new born,12,24,36 months old,were detected for further comparison in order to see the difference between the two groups of children.Results In the new born period,TSH level in case group was significantly higher than that in control group while Serum FT4 and FT3 levels in case group were lower than those in control group.TSH,FT4 and FT3 levels were no significantly different when children were 12,24,36 months between the two groups.Intelligence in case group was significantly lower than that in control group in each period studied respectively.Physical development in case group was lag behind than that in control group in each period,too(P0.05).Conclusion Maternal subclinical thyroid abnormality during early and midtrimester pregnancy may have adverse effects on their children′s intellectual and physical development and may result in temporarily abnormal thyroid function.Therefore,it is necessary to screen and actively treat subclinical thyroid abnormality at early and medium stage of pregnancy and strengthen the management of pregnant women with thyroid disease and their children.
