晚期癌性疼痛患者5种给药方法的药物经济学分析

摘　　要：目的:对我院肿瘤晚期300例重度疼痛患者进行疼痛治疗、病例追踪、疗效观察及用药后的经济学评价。方法:300例患者选择性分为5组,分别给予盐酸布桂嗪片起始剂量(A组)1次30 mg,口服,每6 h给予1次;盐酸吗啡缓释片初始剂量(B组)1次30 mg,口服,每12 h给予1次;硫酸吗啡缓释片初始剂量(C组)1次30 mg,口服,每12 h给予1次;芬太尼透皮贴剂起始剂量(D组)4.2 mg,每72 h给予1贴;盐酸丁丙诺啡舌下片初始剂量(E组)0.4 mg,每8 h给予1次。结果:5组疼痛缓释率分别为45.24%、90.70%、91.78%、92.60%、73.34%,成本分别为106.92、915.00、832.44、1 379.20、891.00元。治疗中出现胃肠道反应如便秘、恶心、呕吐及其他反应者给予对症处理。结论:C组在临床疗效及成本-效果最优。OBJECTIVE： To treat the cancerous pain of 300 patients with advanced cancer and severe pain,follow the cases,observe the effectiveness of treatment and evaluate the pharmacoeconomic effectiveness.METHODS： A total of 300 cancer patients enrolled were assigned to receive one of the following therapies： Bucinnazine Hydrochloride Tablets（group A） orally at an initial dose of 30 mg,q6h;Morphine hydrochloride sustained-release tablets（group B） orally at an initial dose of 30 mg,q12h;Morphine sulfate sustained-release tablets（group C） orally at an initial dose of 30 mg,q12h;Fentanyl transdermal system（group D） at an initial dose of 4.2 mg（patches were replaced in 72 hours）,and Buprenorphine hydrochloride sublingual tablets（group E） at an initial dose of 0.4 mg,q8h.RESULTS： The pain relief rates in the 5 groups were 45.24%,90.70%,91.78%,92.60% and 73.34%,respectively,and the costs of the 5 groups were 106.92 Yuan,915.00 Yuan,832.44 Yuan,1 379.20 yuan and 891.00 Yuan,respectively.Patients with symptoms of gastrointestinal tract such as constipation,nausea and vomiting and other toxic reactions were treated symptomatically.CONCLUSION： Group C is the best one among the 5 groups in terms of clinical efficacy and cost-effectiveness.
