摘 要:目的:研究不同糖代谢状态人群糖尿病视网膜病变(DR)的检出率,并探讨其相关危险因素。方法:选取2011年8月至12月在大连市中心医院内分泌科接受糖尿病社区普查并完成免散瞳眼底照相的居民,按登记序列号顺序抽取糖耐量正常者499例、糖耐量异常者490例、糖尿病病程<10年者499例、糖尿病病程≥10年者319例,观察各人群视网膜病变的检出率;应用多因素Logistic回归分析2型糖尿病社区居民的年龄、病程、体质量指数(BMI)、血糖、血压、血脂等11项指标与DR的相关性。结果:糖耐量正常组和糖耐量异常组视网膜病变检出率分别为0.40%、0.41%(P>0.05);糖尿病病程<10年组DR检出率为7.21%,病程≥10年组为26.65%(P<0.05)。与非DR人群比较,DR患者的年龄、糖尿病病程、空腹血糖、餐后2 h血糖、糖化血红蛋白(HbA1c)明显升高,BMI明显降低(均P<0.05),其中糖尿病病程及HbA1c与DR呈显著正相关。结论:糖尿病病程≥10年的患者视网膜病变检出率最高。病程及HbA1c是DR的独立危险因素。Objective: To study the detection rate of diabetic retinopathy (DR) in different glucose metabolism status and to explore its risk factors. Methods: Community residents accepted the diabetes survey in Department of Endocrine, Dalian Municipal Hospital from August to December 2011 were enrolled in this study. Non-mydriatic fundus camera examination was performed in 499 cases with normal glucose tolerance, 490 cases with impaired glucose tolerance, 499 cases with duration of diabetes less than 10 years and 319 cases with diabetes more than 10 years, and the rates of DR in these groups were analyzed. Multivariate Logistic regression analysis was used to analyze the correlation between DR and 11 indices of type 2 diabetes age, disease duration, body mass index (BMI), blood glucose, blood pressure, and others. Results: The detection rates of DR were: 0.40% in normal glucose tolerance group, 0.41% in impaired glucose tolerance group, 7.21% in duration of diabetes less than 10 years group and 26.65% in diabetes greater than 10 years group. Compared with no retinopathy group, age, duration of diabetes, fasting plasma glucose (FPG), postprandial 2 hours plasma glucose(2hPG), glycated hemoglobin Alc(HbAlc) of retinopathy group were significantly higher, while BMI was significantly lower (P〈0.05). Duration of diabetes and HbAlc had a significant positive correlation with DR. Conclusions: Group with duration of diabetes greater than 10-year had the highest positive rate of DR. Duration of diabetes and HbAlc were independent risk factors for DR.
