硬膜外腔注射不同糖皮质激素对大鼠肾上腺皮质功能及形态学的影响

摘　　要：目的:研究临床常用激素泼尼松、地塞米松、及曲安奈得硬膜外腔注射对SD大鼠肾上腺皮质功能及形态学的影响。方法:雄性SD大鼠60只,随机分为泼尼松组、地塞米松组、曲安奈得组、对照组和正常组5组。每组12只。除空白组外其余各组硬膜外腔给药每周1次,连续注射3周,第4周取血,测血清皮质醇浓度,并取肾上腺制作石蜡切片,苏木精-伊红染色观察测量肾上腺皮质厚度及束状带所占的比例。结果:泼尼松组、对照组和正常组均分别与地塞米松组、曲安奈得组比较,血清皮质醇比较差异有统计学意义(P<0.05)。各组的肾上腺组织苏木精-伊红染色观察测量显示,地塞米松组、曲安奈得组的束状带所占皮质的比例与泼尼松组、对照组和正常组相比下降明显(P<0.01)。结论:硬膜外腔注射长效激素地塞米松和醋酸曲安奈得有可能会引起雄性SD大鼠肾上腺皮质功能减退并引起肾上腺形态学上的萎缩改变,而使用中效激素醋酸泼尼松不会引起此类变化。Objective：To study the effects of epidural injection of prednisone, dexamethasone and triamcinolone acetonide on adrenocortical function and morphology in Sprague-Dawley （SD） rats. Methods：A total of 60 male SD rats were randomly divided into 5 groups, 12 in each group. Rats in Group 1 were treated with epidural injection of prednisone, Group 2 with dexamethasone, and Group 3 with triamcinolone acetonide. Group 4 and Group 5 were control group and normal group. All groups were given epidural injection once a week for three consecutive weeks. Blood samples were drawn in the fourth week for detection of serum corticosteroid concentration. Adrenal cortex was taken to measure adrenal cortex thickness and the proportion of fascicular zone by HE staining. Results： There was statistical difference in serum eorticosteroid between Group 1, Group 4, Group 5 and Group 2 and Group 3 （P〈0.05）. HE staining showed that the proportion of fascicular zone in adrenal cortex in Group 2 and Group 3 was significantly decreased compared with Group 1, Group 4 and Group 5 （P 〈0.01）. Conclusion： Epidural injection of long-acting dexamethasone and triamcinolone acetonide in male SD rats may cause adrenal insufficiency and cause adrenal atrophy, but middle-acting hormones such as prednisone does not cause such changes.
