摘 要:目的了解ICU病区非发酵革兰阴性杆菌的分布特点及耐药性,为临床医师抗感染治疗提供实验室依据。方法回顾分析2010年6月至2011年12月医院ICU分离的366株病原菌的分布及耐药,陆分析,VITEK-60全自动微生物分析仪鉴定菌株,K—B纸片扩散法测定细菌的耐药性。结果366株病原菌中非发酵革兰阴性杆菌254株,检出率为69.4%,以铜绿假单胞菌菌(PAE)、鲍氏不动杆菌(ABA)和嗜麦芽寡氧单胞菌(PMH)为主,所占比率分别为33.3%、29.8%、5.19%。痛原菌PAE和ABA对多粘菌素B耐药性较低,耐药率分别为6.5%和9.17%,对头孢哌酮/舒巴坦的耐药率均控制在35%以下,对其他常用抗菌药物表现出严重耐药性。结论非发酵革兰阴性杆菌为ICU感染的重要病原菌,且耐药性严重,应注重细菌耐药性的监测,控制ICU病区感染。Objective To study the distribution and and drug resistance of nonfermentativc gram- negative bacilli in ICU, and to provide evidence for clinical reasonable use of antibiotics. Methods 366 stains of pathogens from ICU were restropectively analyzed and obtained the chance trend of the distribution and drug resistance. The VITEK - 60 system and identification cards were used to identify the stains. The resistance testing was performed by K - B method. Results 254 nonfermentative gramnegative bacillis were identified among 366 stains of pathogens (69.4 % ). Pseudomonas aeruginosa(PAE), Acinctobacter baumannii(ABA) and Stenotrophomonas maltophilia(PMA) accounted ibr 33.3 %, 29.8 %, and5. 19 %. The PAE and ABA showed lower drug resistance rate in polymyxin B (6.5 % and 9.17 % ), in cefoperazone/sulbactam also lower( 〈 35 % ), but PAE and ABA showed higher drug resistance rate in other drug. Conclusion The nonfermentative gram - negative bacilli are the major pathogens in ICU and show serious drug resistance. It is urgent to control file spead of infections and monitor bacterial resistance in ICU.
