摘 要:目的探讨血清色素上皮源性因子(sPEDF)与2型糖尿病(T2DM)胰岛素抵抗的关系。方法采用酶联免疫法检测176例受试者[糖调节正常组(NGT)52例,T2DM 124例]的sPEDF水平,同时检测空腹C肽(FCP)、2小时C肽(2h CP)糖化血红蛋白(HbA1c)、血糖、血脂、高敏C反应蛋白(hs-CRP)等生化指标的水平。结果 (1)T2DM组sPEDF水平显著高于NGT组[2893.45(1641.91~5533.13)μg/L vs 2672.95(1486.54~4039.13)μg/L(P<0.05)];(2)按sPEDF水平四分位分组,上四分位点的胰岛素抵抗指数(HO-MA-IR)值显著高于下四分位点者[1.90(1.60~2.70)vs 1.45(0.90~2.10)(P<0.01)];(3)单变量相关关系分析显示:sPEDF水平与HOMA-IR、SBP、DBP、FCP、hs-CRP呈显著正相关(r=0.206,0.210,0.170,0.213,0.221,P<0.05或0.01);(4)逐步多元回归显示HOMA-IR(β=0.174,P<0.05),SBP(β=0.189,P<0.05)是sPEDF的独立影响因素。结论中国人群中2型糖尿病患者sPEDF水平显著升高,与胰岛素抵抗指数显著独立相关。Objective Objective To explore the relations between serum epithelium-derived factor and insulin resistance in type2 diabetes mellitus.Methods Using enzeyme-linked immunosorbent assay(ELISA) to detect serum PEDF in 52 normal glucose tolerance people(NGT) and 124 type 2 diabetes mellitus(T2DM) patients.FCP,2hCP,HbA1c,blood glucose,blood lipid and highly sensitive C-reactive protein(hs-protein) were simultaneously determined.Results (1) The serum PEDF(sPEDF) levels of T2DM were significantly higher than of NGT group[2893.45(1641.91-5533.13) μg/L vs 2672.95(1486.54-4039.13) μg/L(P0.05)].(2) Quartiles of the concentration of sPEDF were computed.Compared with those in the lowest quartile of sPEDF,type 2 diabetes mellitus patients in the highest quartile had higher HOMA-IR values [1.90(1.60-2.70) vs 1.45(0.90-2.10)(P0.01)].(3) Bivariate correlation showed that sPEDF was positively correlated with HOMA-IR,SBP,DBP,FCP,hs-CRP(r=0.206,0.210,0.170,0.213,0.221,P0.05 or 0.01).(4) Multiple linear regression analysis revealed that HOMA-IR(β=0.174,P0.05),SBP(β=0.189,P0.05) were the independent factors of sPEDF.Conclusion The serum PEDF level of Chinese population with type 2 diabetes increased significantly,and the serum PEDF level is independently related to HOMA-IR.
