痔上黏膜环切吻合术前应用帕瑞昔布钠的镇痛效果观察

摘　　要：目的观察痔上黏膜环切吻合术(Procedure for prolapsing haemorrhoids,PPH)前应用帕瑞昔布钠的镇痛效果。方法将60例在骶管阻滞下行PPH的患者按随机数字表法分成试验组和对照组各30例。试验组麻醉前15分钟静脉注射2ml帕瑞昔布钠40 mg,对照组麻醉前15分钟静脉注射生理盐水2 ml。术后采用视觉模拟评分(visual analogue scale,VAS)评价疼痛情况,并记录要求使用镇痛药的患者例数。结果术后2、4、8、12、24小时试验组VAS评分显著低于对照组,术后使用镇痛药患者例数明显少于对照组,差异均有统计学意义(P<0.05);两组术后24小时疼痛程度比较,差异有统计学意义(P<0.05)。结论帕瑞昔布钠超前镇痛可明显减轻PPH患者术后疼痛。Objective To investigate the preoperative analgesic effect of Parecoxib Sodium on pain after procedure for prolapsing haemorrhoids（PPH）. Methods Sixty patients underwent PPH were randomly assigned to the controlled group（n = 30） or the test group treated with Parecoxib Sodium（n = 30）.In the test group Parecoxib Sodium 40 mg/2 ml was injected intravenously 15 minutes before caudal block,and Sodium Chloride Physiological Solution 2 ml in the control group.Visual Analogue Scale（VAS） was used to evaluate postoperative pain.Patients who used analgesics were recorded. Results Compared with the control group,the score of VAS in the test group was significantly lower（P 0.05）.There were much less patients who need analgesics after surgery in the test group（P 0.05）. Conclusions Preoperative administration of Parecoxib Sodium can effectively relieve postoperative pain after PPH.
