肺炎克雷伯菌中Ⅰ类整合子与qacE△1-sul1基因的研究

摘  要:目的了解医院产超广谱β-内酰胺酶(ESBLs)肺炎克雷伯菌Ⅰ类整合子和qacE△1-sul1耐消毒剂基因的携带情况。方法收集临床住院患者分离出的产ESBLs肺炎克雷伯菌108株;采用K-B法测定抗菌药物的敏感性;聚合酶链反应(PCR)检测Ⅰ类整合子和qacE△1-sul1基因。结果Ⅰ类整合子基因总检出率为53.70%,qacE△1-sul1基因总检出率为63.89%;产ESBLs肺炎克雷伯株Ⅰ类整合子阳性株除对氨苄西林和哌拉西林的耐药率与Ⅰ类整合子阴性株差异无统计学意义以外,对其他抗菌药物的耐药率均明显高于整合子阴性株,差异有统计学意义(P<0.05)。结论医院产ESBLs肺炎克雷伯菌Ⅰ类整合子和qacE△1-sulⅠ耐消毒剂基因携带率均较高,应进一步加强Ⅰ类整合子和耐消毒剂基因菌株的流行病学监测,并根据耐消毒剂状况,在消毒剂的使用上作出相应的调整。OBJECTIVE To investigate the presence of classⅠintegron and qacE△1-sul1 gene in extended-spectrum β-lactamase(ESBLs)-producing Klebsiella pneumoniae isolates from Yiwu.METHODS We isolated and identified 108 strains of ESBLs-producing K.pneumoniae isolates from clinical inpatients' samples.We used K-B method to determine antimicrobial susceptibility,and detected the class Ⅰintegron and qacE△1-sul1 gene by polymerase chain reaction(PCR).RESULTS The positive rate of the class Ⅰintegron in the isolates was 53.70%,and the detection rate of qacE△1-sul1 gene was 63.89%.The antibiotic resistance of the integron-positive strains was significantly higher than that of the integron-negative ones except ampicillin and piperacillin,the difference was statistically significant(P〈0.05).CONCLUSION The positive rate of classⅠintegron and qacE△1-sul1 gene in K.pneumoniae is so high that we should detect the class Ⅰintegron and disinfectant-resistance genes more frequently,and modulate the type of disinfectants on the basis of the condition of disinfectant-resistance.
