糖尿病视网膜病变患者血清同型半胱氨酸、白细胞介素-6和肿瘤坏死因子-α的检测意义

摘　　要：目的研究2型糖尿病视网膜病变（DR）患者血清同型半胱氨酸（Hcy）、白细胞介素-6（IL-6）和肿瘤坏死因子．仪（TNF—α）的水平变化及临床价值。方法通过眼底血管荧光造影、血糖和眼底检查选择糖尿病视网膜病变患者（DR组）50例，单纯糖尿病无视网膜病变患者（NDR组）30例，另外选择健康体检者30例作对照（对照组），对血清Hcy、IL-6和TNF-α进行检测并比较。结果DR组和NDR组患者血清Hcy、IL-6和TNF—α水平均明显高于正常对照组（t=4．97，5．23，4．47，4．51，5．11，4．22，P〈0．05），DR组血清Hcy、IL-6和TNF-α水平均高于NDR组（t=3．98，4．12，4．21，P〈0．05）。结论Hcy、IL-6和TNF-α可能参与了糖尿病视网膜病变的发病机制，提示炎性反应可能在糖尿病视网膜病变的发展中起重要作用。Objective To investigate clinical value and the changes of homocysteine （Hcy）, in- terleukin-6 （IL-6） and tumor necrosis factor-α （TNF-α） levels in type 2 diabetic patients with diabetic retinopathy. Methods All the patients had routine FFA and examination of blood sugar and fundus, 50 patients with diabetic retinopathy（ DR group） and 30 patients with normal fundus （NDR group） were selected. Thirty healthy subjects were selected as control group. The serum levels of Hcy、IL-6 and TNF-α were detected and compared. Results The serum levels of Hey, IL-6 and TNF-α of DR group and NDR group were obviously higher than those of control group （t = 4.97, 5.23,4.47,4. 51,5.11,4.22, P 〈 0. 05 ）. The serum levels of Hcy 、 IL-6 and TNF-α of DR group were higher than those of NDR group （t =3.98,4. 12,4. 21 ,P 〈 0.05）. Conclusions Hcy、IL-6 and TNF-α may take part in the pathogenesis of diabetic retinopathy, indicating that inflammatory reaction may play an important role in the development of DR.
