预防椎板切除术后硬膜外瘢痕的实验研究

摘　　要：目的　探讨不同剂量非甾类消炎药双氯芬酸钠在预防椎板切除术后硬膜外瘢痕形成中的作用。　方法　建立 45只新西兰大白兔L5 全椎板切除实验模型 ,随机均分为对照组、大剂量和小剂量用药组。通过粘连韧度、组织学、电镜观察和增强MRI扫描等手段 ,检测不同剂量双氯芬酸钠对椎板切除术后硬膜外瘢痕的影响。　结果　大剂量组 (10mg·kg 1 ·d 1 )和小剂量组(5mg·kg 1 ·d 1 )硬膜外瘢痕形成较对照组 (未用药 )少 ,创伤后炎性反应抑制明显 ;大剂量药物较小剂量药物作用更强 ,但两组间副作用无明显差别。Objective To observe the effects of different doses of diclofenac sodium on epidural scar after laminectomy. Methods A total of 45 New Zealand rabbits were established as the experimental models undergoing L 5 laminectomy, and the effects of diclofenac sodium on epidural scar after laminectomy were observed with historical analysis, electronic microscopy, enhanced MRI film, et al. Results The formation of epidural scar and inflammatory reaction after laminectomy in the groups of large and small doses of diclofenac sodium were less than those of the control group. The suppression of epidural scar formation was more obvious in the group with large dose of diclofenac sodium than that in the group with small dose ones, but there was no statistical difference between the two groups. Conclusions Diclofenac sodium is an effective drug in preventing epidural scar after laminectomy.
