ICU导管相关性感染目标性监测分析

摘　　要：目的了解某院重症监护病房(ICU)医院感染发生情况,尤其是导管相关性感染基线数据,为干预措施的制定提供参考。方法采用前瞻性调查方法对2010年6月-2011年12月入住ICU患者的医院感染发生情况进行目标性监测,并对侵入性操作使用率与相关医院感染发生率进行分析。结果调查期间共监测2741例ICU患者,发生感染198例、307例次,感染率为7.22%、例次感染率为11.20%;同期ICU住院总日数9666d,日感染率为20.48‰,例次日感染率为31.76‰;经ASIS调整后的日感染率为6.86‰,日感染例次率为10.64‰;导尿管、中心静脉插管及呼吸机的使用率分别为96.39%、58.66%和50.77%,使用导管所致相关泌尿道、血流感染和呼吸机相关性肺炎分别为3.11‰、5.47‰和20.58‰。结论 ICU医院感染发病率和导管相关性感染率较高,必须制定相应感染控制预防措施,使其相应的感染得到有效控制。OBJECTIVE To explore the incidence of healthcare-associated infections（HAI）,particularly the catheter-associated infections in ICU,to provide reference for constituting the intervention measures.METHODS The nosocomial infections of ICU patients from Jun 2010 to Dec 2011 were investigated using the method of target monitoring.The incidence of HAI and DA was monitored and analyzed.RESULTS Totally 198 of 2741（7.22%）patients developed 307（11.20%）cases of HAI;of totally 9666 days of ICU stay,the nosocomial infection rate per day was 20.48‰.After adjusted with ASIS method,the daily HAI rate and nosocomial case infection rate were 6.86‰ and 10.64‰ respectively.The utilization rates of urinary catheter,central venous catheter and ventilator were 96.39%,58.66% and 50.77% with associated urinary tract infections（3.11‰）,bloodstream infections（5.47‰）,and ventilator-associated pneumonia（20.58‰） respectively.CONCLUSION The rates of HAI and DA infection in ICU are high,it is necessary to develop the corresponding prevention and control measures to effectively control the infections accordingly.
