2009-2011年医院感染现患率调查与分析

摘　　要：目的了解医院感染现患率及动态变化,增加感染监测工作的透明度,为制定有效地预防控制措施提供科学依据。方法采取床旁调查和病例调查的方式,对2009年10月14日、2010年5月27日及2011年7月21日(0∶00-24∶00)所有住院患者进行医院感染现患率调查,将调查结果录入分析。结果 3次调查平均医院感染现患率为3.19%,平均例次感染率为3.28%,发生医院感染现患率前5位的科室是ICU、神经外科、神经内科、心血管内科及血液风湿科;医院感染主要分布在下呼吸道;抗菌药物使用率平均为25.59%,呈现逐年升高的趋势;使用抗菌药物病原学培养送检率平均54.74%,共检出病原菌66株,革兰阴性菌为主。结论 3次现患率调查,阐明了医院感染的基本情况及发展动态,发现了医院感染高危科室和感染主要部位,对于今后制定感染控制措施、加强医院感染监督检查具有重要作用。OBJECTIVE To understand the prevalence rate of nosocomial infections and its dynamic change,improve the transparency of the surveillance of infections so as to provide basis for the development of prevention and control countermeasures.METHODS By the means of bedside investigation and the review of medical records,the prevalence rates of the hospitalized patients who enrolled in hospital on Oct 14 2009,May 27 2010,and Jul 21 2011 were analyzed.RESULTS The average prevalence rate of nosocomial infections was 3.19%,the average case-time infection rate was 3.28%;the top five prevalence rates were in ICU,neurosurgery department,neurology department,department of cardiovascular medicine and department of blood rheumatism;the lower respiratory tract was the major infection site;the average utilization rate of antibacterial agents was 25.59%,presenting as an upward tendency;the submission rate of etiological culture was 54.74%;there were 66 strains of pathogens isolated,among which gram-negative bacteria were dominat.CONCLUSION Three times of investigation of prevalence rates has demonstrated the general status and the dynamic change of nosocomial infections,identified the high-risk departments and the infection sites,which plays an important role in the development of control measures and the supervision of nosocomial infections.
