参苓白术散加减治疗化疗后白细胞下降42例临床研究

摘　　要：目的观察参苓白术散加减治疗妇科恶性肿瘤化疗后白细胞(WBC)下降的临床疗效及其对患者生活质量的影响。方法将84例妇科恶性肿瘤化疗后WBC下降患者随机分为治疗组与对照组,每组42例,对照组患者给予利血生与鲨肝醇进行常规治疗;治疗组在对照组基础上给予参苓白术散加减,两组均以4周为1个疗程。2个疗程后评定疗效并观察两组治疗前后外周血细胞包括WBC、红细胞(RBC)、血红蛋白(HGB)、血小板(PLT)、淋巴细胞(LYM)、中性粒细胞(GRAN)、血小板比率(PLT%)、淋巴细胞比率(LYM%)、中值细胞比率(MON%)、中性细胞比率(GRAN%)等指标的变化及Spitzer生存质量指数评定量表评分(Spitzer评分)、癌症康复评价简表(CARES-SF评分)情况。结果两组患者治疗后WBC、RBC、HGB、PLT、LYM、GRAN、PLT%、LYM%、MON%、GRAN%等各项指标与治疗前比较均有明显改善(P<0.05);治疗后治疗组WBC、HGB、LYM、MON%、GRAN%改善程度明显优于对照组(P<0.05)。治疗组总有效率为90.48%,对照组总有效率为73.81%,治疗组明显优于对照组(P<0.05)。与治疗前比较,两组治疗后Spitzer评分明显提高(P<0.05)、治疗组治疗后CARES-SF评分明显降低(P<0.05);与对照组治疗后比较,治疗组Spitzer评分、CARES-SF评分差异均有统计学意义(P<0.05)。结论参苓白术散加减治疗妇科恶性肿瘤化疗后WBC下降疗效显著,可明显提高患者生活质量。Objective To observe the therapeutic effect of Shenling Baizhu Powder （Ginseng, Poria, and White Atractylodes Powder） on leucopenia after chemotherapy and improvement of quality of life （QOL）. Methods Eighty four leucopenia patients after chemotherapy of gynecologic malignancies were randomic, ed into treatment group and control group, with 42 in each. The control group was given leucogen and batilol and the treatment group was given Shenling Baizhu Powder in addition. One treatment course lasted for 4 weeks. The therapeutic effect was evaluated after two treatment courses. White blood cell （WBC）, red blood cell （RBC）, hemoglobin （HGB）, platelet （PLT）, lymphocytes （LYM）, granulocytes （GRAN）, PLT%, LYM%, monocytes （MON）%, GRAN%, Spitzer scores and San Francisco Comprehensive Approaches to Raising Educational Standards （CARES-SF） scores were observed. Results After treatment, WBC, RBC, HGB, PLT, LYM, GRAN, PLT%, LYM%, MON% and GRAN% were significantly improved in both groups （P〈0.05）. Comparing with the control group, WBC, HGB, LYM, MON% and GRAN% were improved significantly in the treatment group （P〈0.05）. The total effective rate of the treatment group and the control group was 90.48; and 73.81% respectively, with a significant difference between groups （P〈0.05）. The main symptoms and signs were relieved and the Spitzer scores and CARES-SF scores were improved significantly in both groups after treatment （P〈0. 05）. Comparing with the control group, the Spitzer scores and CARES-SF scores were significantly improved in the treatment group after treatment （P〈0. 05）. Conclusion Shenling Baizhu Powder is effective for leucopenia after chemotherapy of gynecologic malignancies and could significantly improve QOL of the patients.
