摘  要:目的分析不同病理分级脑胶质瘤超声造影参数与微血管密度之间的关系,探讨术中超声造影在评价不同病理分级脑胶质瘤血管生成中的价值。方法选取38例脑胶质瘤患者行术中超声造影检查,切除的瘤体应用免疫组化法检测CD34的表达,并记录其微血管密度(MicroVesselDensity,MVD),定量分析不同病理分级脑胶质瘤造影参数与微血管密度的相关性。结果高级别胶质瘤(High-Grade Glioma,HGG)的MVD为(57.23±14.99)条/HP,低级别胶质瘤(Low-Grade Glioma,LGG)的MVD为(28.88±12.36)条/HP,两组间比较差异有统计学意义;瘤体组织绝对峰值强度(Absolute Peak Intensity,API)与MVD间呈正相关(r=0.886,P<0.05),达峰时间(Time To Peak,TTP)与MVD间呈负相关(r=-0.715,P<0.05)。结论超声造影参数API和TTP与MVD有良好的相关性,术中超声造影可以间接反映瘤体的血管生成情况,有助于肿瘤切除前初步判断其病理分级及指导术后治疗。Objective To investigate the value of intraoperative contrast-enhanced ultrasound in evaluating angiogenesis of cerebral gliomas by analyzing the relations between contrast-enhanced ultrasound perfusion parameters and tumor microvessel density(MVD) of different pathologic grades of cerebral gliomas.Methods 38 cerebral gliomas were examined by contrast-enhanced ultrasound,the microvessels density were counted by immunohistochemical staining with anti-CD34,and the correlation between contrast-enhanced ultrasound perfusion parameters and tumor microvessel density of different pathologic grades of cerebral gliomas was analyzed.Results The mean of MVD in the group of the low and high grade cerebral gliomas was 28.88±12.36 and 57.23±14.99,and the MVD was statistically significant(P0.05).The value of the absolute peak intensity was positive correlated to the level of MVD in gliomas(r=0.886,P0.05).The time to peak was negative correlation with MVD(r=-0.715,P0.05).Conclusion According to significant correlation between contrast-enhanced ultrasound perfusion parameters and MVD,intraoperative contrast-enhanced ultrasound could indirectly reflect the information of gliomas vascularity,which was help to grade cerebral gliomas and guide postoperative treatment.
