摘 要:目的研究电化学发光法检测抗CCP抗体的临床应用价值。方法对48名临床确诊为类风湿关节炎的患者、22例其他疾病患者、20例健康成人用电化学发光法检测抗CCP抗体,并同时用免疫散射速率法检测RF。结果抗CCP抗体对RA诊断敏感性为72.92%,特异性为97.61%,阳性预测值97.22%,阴性预测值75.92%;RF对RA诊断的敏感性为85.42%,特异性为71.43%,阳性预测值77.36%,阴性预测值81.08%;抗CCP抗体与RF联合平行试验对RA诊断的敏感性为96.05%,特异性69.72%,阳性预测值78.18%,阴性预测值85.71%。结论在RA患者中,抗CCP抗体较RF有更高的特异性。电化学发光法检测抗CCP抗体与RF联合检测可提高诊断RA的敏感性,为患者早期诊断提供实验室依据。Objective To determine the clinical significance of anti-cyclic citruUinated peptide antibody. Methods A-CCP antibody was detected by the electrochemical luminescence method and rheumatoid factor(RF) by the Immune scattering rate method in 48 cases o f RA, 45 cases of other rheumatism disease and 20 health adults, respectively. Results The sensitivity and specificity of A-CCP antibody in RA were 72.92% and 97.61%, PPV(positive predictive value) was 97.22%, NPV(negative predictive value) was 75.92%, The sensitivity and specificity of RF were 85.42% and 71.43%, PPV was77.36% and NPV was 81.08%, respectively. The sensitivity and specificity of A-CCP antibody and RF joint parallel tests in RA were 96.05% and 69.72%, PPV was 78.18%, NPV was 85.71%, respectively. Conclusion A-CCP antibody has higher specificity than RF in RA.A-CCP antibody and RF joint parallel tests can improve the sensitivity of the RA.
