摘  要:目的研究血清胱抑素C(Cyc-C)、尿微量白蛋白(mALB)浓度在早期肾病诊断中的临床意义。方法选择汤阴县人民医院肾病科确诊为肾功能损伤患者74例,同期选择在本院健康体检的年龄相当正常人62例作为正常对照组,检测血清Cyc-C、尿素氮(Urea)、血肌酐(Scr)及mALB浓度,并计算内生肌酐清除率(Ccr)。结果肾病患者早期肾功能正常和肾功能代偿期血清Cyc-C与正常对照组比较有统计学差异(P<0.01或P<0.05);肾功能失代偿期及肾功能衰竭期血清Cyc-C、Urea、Scr及mALB与正常对照组比较有统计学差异(P<0.01或P<0.05);血清Cyc-C与Ccr呈负性相关(P<0.001);肾病患者血清Cyc-C及尿mALB单项检测阳性率高于Scr及mALB;多项联合检测能提高肾病的早期诊断。结论肾病早期血清Cyc-C能准确反映肾小球滤过率及肾功能的变化,血清Cyc-C和mALB联合检测能提高早期肾损伤的敏感性,对早期肾病的诊断具有重要的临床意义。Objective Investigative the clinical significance of serum cystatin C(Cyc-C), urinary albumin(of mALB) concentration in the diagnosis of early kidney disease. Methods Select confirmed 74 cases of patients with renal injury inTangyin People's Hospital Nephrology, and select 62 cases of healthy and age-matched normal as normal control group at the same period in the hospital, to detect Cyc-C, serum urea nitrogen(Urea), serum creatinine(Scr) and mALB concentration, and calculate the endogenous creatinine clearance rate(Ccr). Results The Cyc-C of early renal disease patients with normal renal function and renal function in decompensated serum has statistical differences compared with the normal control group(P〈0.01 or P〈0.05), The Cyc-C, Urea, Scr and mALB of Loss of renal function decompensated renal function failure of serum has statistical differences compared with the normal control group(P〈0.01 or P〈0.05), The Cyc-C was inversely proportional to Ccr, the individual testing positive rate of Cyc-C and urine mALB was higher than Scr and mALB In nephrotic serum, A number of joint detection can improve the early diagnosis of nephropathy. Conclusions The Cyc-C of Nephropathy serum can accurately reflect the changes in glomerular filtration rate and renal function, Joint detection between Cyc-C and mALB of serum can improve the sensitivity of early renal damage and has important clinical significance for the diagnosis of early kidney disease.
