显微线圈在颞颌关节的应用价值的初步研究

摘  要:目的初步探讨磁共振显微线圈(microscopy coil,MC)MR高分辨率成像技术在颞颌关节的应用价值。方法应用优化后的PDWI序列分别用显微线圈和头颈联合线圈颞颌下关节扫描,分别对23名正常志愿者双侧颞颌下关节闭口位磁共振成像,并由3名高年资医师分别对两组线圈图像进行解剖结构的显示进行评分。结果显微线圈在对颞颌下关节盘、关节窝、髁突骨质形态、关节间隙、盘周组织、翼外肌显示具有一定优势(P<0.001);头颈联合线圈较利于外耳道及乳突结构解剖形态显示较好,两者比较差异有统计学意义。结论优化后的PDWI序列在显微线圈成像能较好显示颞颌下关节解剖细节,获得高空间分辨力的优质图像。高分辨MR显微成像技术的应用,能提高颞颌下关节解剖精细结构的显示能力,从而提高诊断。Objective Preliminary Study the application value of magnetic resonance microscopy coil (MC), MR high- resolution imaging technique in the temporal submandibular joint. Methods Using the microscopy coil and Head- Neck joint coil with optimized PDWI sequence, bilateral temporal submandibular joint of 23 normal volunteers were experienced closed-bit magnetic resonance imaging. Both sets of coil images were scored by three experienced radiologists in displaying anatomical structure. Results Microscopy coil has certain advantages in displaying the temporal submandibular articular disc, articular fossa, the morphology of condylar bone, joint space, the disc tissue and lateral pterygoid muscle(P〈0.001). Head-Neck joint coil is more conducive to show the external auditory canal and mastoid structure, the difference between two groups was statistically significant. Conclusion The microscopy coil imaging with the optimized PDWI sequence is more conducive to show anatomical details of the temporal submandibular, and acquire high-quality images with high spatial resolution. High-resolution MR microscopy imaging technology can better display anatomical fine structure of the temporal submandibular, thereby increasing the level of diagnosis.
