52例结节性甲状腺肿患者的术式选择

摘　　要：目的探讨外科治疗结节性甲状腺肿手术方式的选择。方法笔者将2011年6月至2012年6月收治的52例结节性甲状腺肿患者根据不同的手术方式分为三组,单纯甲状腺肿块剜除术17例,甲状腺次全切术组18例,甲状腺近全切术组17例,比较三组患者术后的临床疗效,并发症情况和复发率情况。结果三组患者经手术治疗后均有所好转,甲状腺次全切术组总有效率94.1%,显著优于甲状腺肿块剜除术组(58.8%)及甲状腺近全切术组(64.7%),有统计学意义(P<0.05);甲状腺近全切术组并发症发生率为5.5%,显著低于甲状腺肿块剜除术组(23.5%)和甲状腺次全切术组(29.4%),P<0.05;随访3个月后,甲状腺近全切术组并发症发生率为29.4%,甲状腺次全切术组为11.1%,甲状腺肿块剜除术组为5.9%;复发率甲状腺近全切术组为5.9%,甲状腺次全切术组为11.1%,甲状腺肿块剜除术组为23.5%。结论甲状腺近全切术治疗结节性甲状腺肿较为合理,术后患者临床疗效明显,并发症情况发生较少,且复发率较低。Objective To explore the selection of operation for 52 patients with nodular goiter.Methods 52 cases of nodular goiter patients admitted to our hospital from June 2011 to June 2012 according to the different surgical methods,they were divided into three groups.17 cases were of thyroid removal surgery group,18 cases were of thyroid hysterectomy group,and thyroid near total resection group was in 17 cases.The clinical efficacy postoperative complications and recurrence rates of the three groups were compared.Results The three groups of patients after surgery have improved.The total efficiency in thyroid hysterectomy group was 94.1%,was better than that of thyroid removal surgery group（58.8%） and the thyroid near total resection group（64.7%）,there were significant difference（P0.05）.The recurrence rate of thyroid sub-total resection group follow-up 3 months was 5.9%,significantly lower than that of thyroid near total resection group（11.1%） and the thyroid near total resection group（23.5%）.The complication rate for thyroid removal surgery group was also 5.9%,significantly lower than that of thyroid near total resection group（29.4%） and hyroid subtotal resection group（11.1%）.Conclusion Thyroid near total resection treatment of nodular goiter is more reasonable,has a clinical efficacy for postoperative,and with low complications and low recurrence rate.
