打鼾120例多导睡眠图分析

摘　　要：目的　探讨打鼾的危害性及多导睡眠图 (PSG)的临床价值 .方法　分析打鼾者 1 2 0例的 PSG.按呼吸暂停指数(AHI)≥ 5和 <5分为睡眠呼吸暂停综合征 (SAS)和单纯打鼾(SS)两组进行比较 .结果　 SAS和 SS各 6 0例 . SAS组男性比例大于 SS组 (P <0 .0 5 ) ,体质量指数 (BMI)也高 (P <0 .0 1 ) .打鼾者的睡眠质量差 ,两组睡眠效率 (SE)差异不显著 ,但 SAS组睡眠潜伏期 (SL )短 (P<0 .0 1 )、微觉醒 (MA)次数多 (P<0 .日 0 5 )、深睡少 (P<0 .0 1 ) . SAS组睡眠低氧血症发生率和氧减饱和度指数 (DOI)显著高于 SS组 (P<0 .0 1 ) .夜间出现心血管系统异常 6 8例 (5 6 .7% ) ,SAS组显著高于 SS组 (P<0 .0 1 ) .结论　打鼾影响生活质量 ,明显增加心血管系统并发症 .AIM To evaluate the risk of snoring and the clinical value of polysomnograms (PSGs). METHODS 120 snorers were divided into sleep apnea syndrome (SAS) group and single snoring (SS) group according to the apnea/hypopnea index (AHI) ≥ 5 and <5, and their PSGs were analyzed. RESULTS There were 60 cases in SAS group and 60 cases in SS group. Male predominated and the body muscle index (BMI) was greater in SAS than that in SS ( P <0.05 and P <0.01, respectively). The sleep quality of snorers was poor and no significant difference of sleep efficiency (SE) was found between the two groups. However, compared with those in SS group, shortened sleep latency, increased micro arousals (MA) and decreased deep sleep were found in SAS group ( P <0.01, P <0.05 and P <0.01, respectively). The occurrence of nocturnal hypoxemia and the degree of oxygen de saturation index were significantly higher in SAS than those in SS (both P <0.01). 68 cases (56.7%) showed EKG abnormalities and/or hypertension during the night. SAS group had more cases than SS group ( P <0.01). CONCLUSION Serious snoring deteriorates the life quality and increases the cardiovascular system complications. PSG is valuable in the diagnosis and verification of management of snoring.
