摘 要:目的 了解细胞凋亡相关蛋白 Fas和 Fas L 的表达在桥本甲状腺炎发病机制及病理变化中的作用及意义 .方法 采用免疫组织化学方法和图像分析 ,检测 1 7例桥本甲状腺炎患者甲状腺标本中 Fas和 Fas L的表达及分布 .结果 在桥本甲状腺炎的甲状腺滤泡细胞中 Fas和 Fas L的免疫染色阳性率和免疫染色强度均显著高于非毒性甲状腺组 (P<0 .0 1 ) .桥本甲状腺炎中 Fas和 Fas L免疫染色强阳性甲状腺滤泡细胞多分布于浸润淋巴滤泡附近 ,浸润淋巴细胞中 Fas,Fas L 免疫染色较弱 .结论 桥本甲状腺炎中 Fas,Fas L 在甲状腺滤泡细胞中呈有特征性分布的高表达 ,浸润淋巴细胞 Fas,Fas L表达较少 ,提示甲状腺滤泡细胞可能通过自身表达 Fas和Fas L产生凋亡 ,致甲状腺滤泡萎缩、破坏 。AIM To investigate the significance of expression of the apoptosis related proteins Fas and FasL in the pathogenesis and pathological changes in Hashimoto′s thyroiditis (HT). METHODS Expression and distribution of Fas and FasL proteins in thyroid tissues from 17 patients with HT were investigated by immunohistochemical methods. RESULTS The immunostaining positive rates and the intensity of positive immunostaining for Fas and FasL in thyrocytes from HT patients were significantly higher than those from the nontoxic goiter (NTG) patients ( P <0.01). Strongly positive immunostained thyrocytes of HT for Fas and FasL mainly distributed in follicles adjacent to lymphocytic infiltrates. Staining of infiltrating lymphocytes for Fas and FasL was found to be rather weak or even absent. CONCLUSION High characteristic expression of apoptosis related proteins Fas and FasL on thyrocytes from HT is observed but the staining of infiltrating lymphocytes for Fas and FasL is rather weak or even absent. Coexpression of Fas/FasL by thyrocytes in HT can lead to their suicidal and/or fratricidal death (apoptosis), resulting in thyroid follicle atrophy and destruction, which may partly accounts for the diffuse lymphocytic infiltration in HT.
