2型糖尿病患者血清IGF-1及IGFBP3水平的检测与分析

摘　　要：目的检测2型糖尿病（T2DM）患者血清胰岛素样生长因子-1（1GF-1）与胰岛素样生长因子结合蛋白3（IGFBP3）水平，并分析注射外源胰岛素及口服降糖药物对二者水平的影响。方法选取2007-2010年天津市人民医院门诊及住院的2型糖尿病患者90例，病程均≥10年，根据治疗方式及注射胰岛素时间分为2型糖尿病口服降糖药物（B）组、注射胰岛素〈5年（≥1年）（C）组和注射胰岛素≥5年（D）组，各30例，并以30例健康查体志愿者为正常对照（A）组。分别运用化学发光免疫分析法（chemiluminescence immunoassay，CI，IA）和酶联免疫吸附试验（enzyme-linkedimmunosorhentassay，EI，ISA）检测各组受试者空腹血清IGF一1和IGFBP3水平，各组问进行统计学分析。结果C组和D组血清IG卜1水平（182．51±59．80）ng／ml，（197．50±92．97）ng／ml与A组和B组（144．44±48．39）ng／ml，（144．15士50．23）ng／m1相比较显著升高，差异有统计学意义（均P〈0．01），而A组与B组、c组与D组之间相比较，差异无统计学意义（均P〉0．05）；各组间血清IGFBP3水平差异无统计学意义（P〉O。05）；D组IGF-1／IGFBP3比值（3．37±1．89）高于A组（2．60±0．69）和B组（2．50±0．69），差异有统计学意义（均P〈0．05）；各组IGF-1与IGBP3相关性分析，二者均呈正相关，A，B，C和I）组r值分别为0．404。0．444，0．473和0．572。结论2型糖尿病患者长期注射外源性胰岛素可导致血清IGF一1水平及IGF-1／IGFBP3比值增加，而口服降糖药物对患者血清二者水平及比值影响较小。Abstract：Objective To detect the serum insulin-like growth factor-1 （IGF-1）and insulin-like growth factor binding protein 3 （IGFBP3） levels in patients with type 2 diabetes mellitus （T2DM）. Then, to analyse the effect of exogenous insulin injec- tions and oral antidiabetic agents on their levels. Methods Selected 90 T2DM patients who had suffered from the disease for more than 10 years between 2007 and 2010. According to the different treatment and insulin injection time of patients, Select- ed 30 patients with oral antidiabetic agents （B group） ,30 patients with insulin injections〈5 years and≥l year （C group） ,30 patients with insulin injections≥5 years （D group）,and 30 healthy controls （A group） were included in this study. Serum IGF-1 and IGFBP3 levels were measured with chemiluminescence Immunoassay （CLIA） and enzyme-linked immunosorbent assay （ELISA） respectively. Analyze the statistics of each group. Results Serum IGF-1 levels in C group （182. 51±59.80） ng/ml and D group （197.50± 92. 97）ng/ml were significantly higher than that of A group （144.44 ±48. 39）ng/ml and B group （144.15±50. 23） ng/ml （ P〈0. 01, respectively） ; and there were no significance in serum IGF-1 levels between A group and B group （P〉0. 05） ,and between C group and D group （P〉0.05）. There were no statistically significant difier- ences among these patients for serum levels of IGFBP3 （P〉0. 05,respectively）,whereas IGF-1/IGFBP3 ratio in D group 3.37±1.89 was higher than that of A group 2.60 ±0.69 and B group 2.50±0. 69 （P〈0. 05, respectively）. The relevant a- nalysis of IGF-1 and IGFBP3 levels in A, B, C and D group showed that they were positively correlated, r were 0. 404, 0. 444,0. 473 and 0. 572,respectively. Conclusion These results suggested that serum IGF-1 levels and IGF-1/IGFBP3 ratio could increase significantly in T2DM patients after long term insulin injections,whereas there were no significance in T2DM patients with oral antidiabeti
