不同原因血小板减少症BAFF和APRIL表达及意义

摘　　要：目的研究B细胞激活因子（BAFF）和增殖诱导配体（APRIL）在不同原因血小板减少症患者中的表达情况及其临床意义。方法应用双抗夹心ELISA方法检测130例患者（其中ITP组35例、淋系血液肿瘤20例、髓系血液肿瘤17例、其他血液病20例和实体肿瘤组38例）以及26名对照组的血清BAFF和APRIL水平，并进行相关分析。结果①ITP组的血清BAFF和APRIL分另lj为3．92土1．88ng／ml和33．92土25．49ng／ml，均明显高于对照组的2．90士0．52ng／ml和17．97土4．96ng／ml（分别为q’=3．12和q’=3．33，均为P〈O．01）。②ITP组血清BAFF和APRIL水平呈正相关关系（r=0．3527，t=2．1651，P＜（0．05）。③淋系血液肿瘤组的血清BAFF和APRIL分别为3．66±1．92ng／ml和38。29士24．99ng／ml也均明显高于对照组（分别为q’=2．99，P〈0．05和q’=4．74，P〈0．01）。④淋系血液肿瘤组的血清BAFF和A-PRIL水平呈正相关关系（r=0．5484，t=2．7838，P〈0．01）。⑤髓系血液肿瘤、其他血液病和实体肿瘤三组患者的血清BAFF和APRIL与对照组比较差异均无统计学显著性意义（均为P〉0．05），并且该三组的BAFF与APRIL均无相关关系（均为P〉0．05）。结论ITP患者和淋系血液肿瘤患者的血清BAFF和APRIL均高表达．BAFF和APRIL可能是ITP等B细胞相关疾病发生机制的重要原因之一。Objective To study the expression and significance of B-cell activating factor（BAFF） and a proliferation-inducing ligand（APRIL） in thrombocytopenic patients of different cause. Methods To test the serum levels of BAFF and APRIL in 130 cases, including 38 cases with idiopathic thrombocytopenic purpur（ITP）, 20 cases with lymphocytic malignant, 17 cases with myelogenous malignant, 20 cases with the other hematological diseases and 38 cases with tumor by ELISA. Results ①The serum levels of BAFF and APRIL in ITP were 3.92＋ 1.88 ng/ml and 33. 92＋25.49 ng/ml respectively,which were higher than control group （2. 90＋0. 52 ng/ml and 17. 974-4.96 ng/ml, q＇ =3.12, P〈0. 01 and q’ =3. 33, P〈0. 01）.② There were positive correlation both BAFF and APRIL in ITP（ r= 0. 352 7, t= 2. 165 1 ,all P〈0. 05）. QThe serum levels of BAFF and APRIL in lymphocytic malignant were 3.66 4- 1.92 ng/ml and 38. 29 4- 24. 99 ng/ml respectively, which were higher than control group （q＇= 2. 99, P^0. 01 and q＇= 4. 74, P〈0. 01 respectively）.④There were positive correlation both BAFF and APRIL in lymphocytic malignant （r= 0. 548 4, t= 2. 783 8, P〈0. 05）. ⑤The serum levels of BAFF and APRIL in other three group patients were no significance compared to control group,and there were no correlation both BAFF and APRIL（all P〉0. 05）. Conclusion The expression of BAFF and APRIL in patients with ITP and lymphocytic malignant, and may be one of factor contributed the ITP and lymphocytic malignant.
