库存血G6PD活性检测在新生儿高胆红素血症换血治疗中的意义

摘　　要：目的探讨新生儿高胆红素血症换血治疗时库存血G6PD活性检测的临床意义。方法根据换入的库存血的G6PD活性检测结果将177例接受换血治疗的高胆红素血症新生儿分为两组：库存血G6PD活性降低组（G6PD-D组）与库存血G6PD活性正常组（G6PD-N组）。比较换血后各时段（O，12，24h）两组患儿的TSB水平及其下降百分比。结果①换入的370袋库存血中，26袋（7．03％）为G6PD活性降低。②两组患儿在换血前及换血后0时TSB水平及其下降百分比差异均无统计学意义（t分别为0．33，0．10和0．04，P〉0．05）；而换血后12h～24h期间，G6PD-D组TSB水平明显高于G6PD）IN组（t分别为4．39和5．63，P=0．00），TSB下降百分比明显小于G6PD-N组（t分别为8．90和11．02，P=0．00）。结论高胆红素血症新生儿换血治疗时输入G6PD活性降低的库存血液将使换血后患儿的胆红素水平下降较慢，建议新生儿换血治疗前对库存血液进行G6PD活性筛查实验。Objective To analyze the clinical significance of bank blood glucose 6-phosphate dehydrogenase（G-6-PD） activity detection during exchange transfusion in hyperbilirubinemia neonates. Methods 177 neonates with hyperbilirubinemia who received exchange transfusion were divided into G6PD-normal group（G6PD-N） and G6PD-decreased group（G6PD-D） ac- cording to the G6PD activity of bank blood. The levels and drop percentage of total serum bilirubin（TSB） at 0, 12 and 24 hours after exchanges were compared between the two groups. Results ①Amongthe 370 bags of bank blood transfused into neonates,26 （7.03%） were G6PD-decreased. ②The difference of TSB levels and the drop percentage between the two groups were not significant（ t= 0. 33,0.10 and 0. 04, P〉0. 05） before exchange and at 0 hour after exchange. However, at 12 hours and 24 hours after exchange,compared with the neonates in G6PD-N group,the neonates in G6PD-D group had a significantly higher level of TSB（t= 4.39 and 5.63, P=0. 00） and lesser drop percentage of TSB（ t= 8. 90 and 11.02, P= 0.00）. Conclusion In neonates with hyperbilirubinemia,exchange transfusion with G6PD decreased bank blood would cause a slower decrease of TSB level after exchange. The results suggest that bank blood should be screened for G6PD activity be- fore being used for exchange transfusion in neonates.
