摘 要:目的 探讨新生儿血清骨代谢指标水平与胎龄和生长指标之间的相关性.方法 取90例新生儿,检测血清骨钙素(OC)、β胶原分解片段(β-CTx)、血钙(Ca2+)、25-羟胆骨化醇(25(OH)D3)和甲状旁腺素(PTH)水平,并对血清骨代谢指标水平与胎龄、体重、身长和头围之间进行相关性分析.结果 90例新生儿血清OC、β-CTx、Ca2+、25(OH)D3、PTH水平分别为(54.5±24.3)μg/L、(1.1±0.4)μg/L、(2.1±0.3)mmol/L、(97.3±47.0)nmol/L和(71.4±57.3)ng/L.新生儿血清骨代谢指标水平与身长和头围之间不存在相关性;血清OC、Ca2+水平与胎龄和体重呈正相关;血清β-CTx水平与胎龄和体重呈负相关;血清PTH水平与胎龄呈正相关,而与体重呈负相关;血清25(OH)D3水平与胎龄和体重之间不存在相关性.结论 新生儿血清骨代谢指标水平主要受胎龄和体重因素的影响,胎龄越小、体重越轻的新生儿越容易发生代谢性骨病,应注意加强代谢性骨病的监测.Objective To explore the relationship between bone metabolism merkers with gestational age and growth indicators in neonates. Methods Concentrations of serum osteoealcin ( OC), beta-crossLaps ( β-CTx), calcium, 25-hydroxycholecalciferol (25 (OH) D3 ) , and parathyroid hormone (PTH) were measured in 90 neonates, and correlation analysis was performed to examine the relationship of bone metabolism markers with gestational age, birth weight, body length, and head eireumference. Results Level of serum OC, β-CTx, Ca2+ , 25 (OH) D3, and PTH were (54.5±24.3) μg/L, (1.1 ±0.4) μg/L, (2.1 ±0.3) mmol/L,(97.3 +47.0) nmol/L, and (71.4±57.3) ng/L, respectively. There were not relationship between bone metabolism markers with body length and head circumference. Levels of serum OC, Ca2+ had positive correlations with gestational age and birth weight. In contrast, levels of serum β-CTx had nega- tive correlations with gestational age and birth weight. Levels of serum PTH were positively correlated with gestational age, but negatively correlated with birth weight. There were no relationships between serum levels of 25 (OH)D3 with gestational age and birth weight. Conclusion Gestational age and birth weight are correlated with bone metabolism in neonates. Neonates with shorter gestational age and lower birth weight may predispose to metabolic bone disorders.
