阑尾周围脓肿不同治疗方法的临床对比研究(附423例临床病例)

摘　　要：目的:对比研究阑尾周围脓肿手术与保守治疗的临床效果。方法:收治阑尾周围脓肿患者423例,对临床资料进行回顾性分析,对比研究保守治疗与手术治疗临床疗效差异。结果:手术组198例,治愈率100%,平均疗程9.56±3.6天,手术并发症发生率5.56%(11/198),其中2例肠瘘,9例切口感染。保守治疗组治愈率76.89%,平均疗程24.7±2.9天。结论:病程在2周以内的阑尾周围脓肿积极采取适当的手术治疗能及早控制炎症和缩短病程;手术治疗临床疗效优于保守治疗(P<0.05)。Objective： Comparative study of effect of operative and conservative treatment of periappendicular abscess. Methods： From 2000 to 2010, admitted to our hospital was a retrospective analysis of 423 cases of periappendiceal abscess treatment,comparative study on effect of conservative treatment and surgical therapy. Results： The surgical group cure rate 100% , course 9.56 ± 3.6 days on average, the complication rate of 5.56% ,of which 2 cases of intestinal fistula and 3 cases of intestinal anastomosis are all occurring in the course of patients with more than 2 weeks. Conservative treatment, cure rate 76. 89% ,an average of 24. 7 -＋2. 9 days of treatment. Conclusion ： Course in less than 2 weeks of appendiceal abscess can be actively take appropriate surgical treatment to control inflammation and shorten course.
