微创后外侧切口全髋置换临床探讨

摘　　要：目的:探讨微创后外侧切口全髋置换的临床疗效。方法:31例(39髋)行后外侧微创切口人工全髋置换治疗,运用传统器械,根据切口长度、手术时间、出血量、假体位置评估。切口长度9.5±3.0cm,其中男15例(19髋),女16例(20髋),年龄58±8.1岁,体重60±22.2kg。结果:手术时间43±11分/髋,出血量380±82ml;1例前倾角稍小,但不影响功能;所有患者随访48～60个月,功能良好;Harris评分:术前40.2±8.0分,术后85.4±9.0分,术前术后P<0.05差异有统计学意义。结论:微创后外侧切口全髋置换,不用专用器械也能获得良好疗效,切口长度主要取决于体重指数。Objective：To explore mini- invasive surgery total hip arthroplasty by posterolateral incision. Method： 31 males （39 hips） and 16 females（20 hips） were accepted by mini - invasive surgery total hiparthroplasty with posterolateral incision, traditional instrument and evaluation according to length of incision, time ofoperation, blood loss and prosthesis position. Result： The time of operation is 43± 11 min/hip, blood loss is 380 ±82 ml ; One patient＇s anteversion of femoral neck is smaller,but the function of the hip is good ; All patients werefollowed up from 48 months to 60 months, the functions were excellent, and the Harris ＇s score is 40. 2 ±8.0 atpreoperation, is 85.4 ±9.0 at postoperation, P 〈 0. 05, and is a statistical significance. Conclusion ： Withposterolateral incision, mini - invasive surgery total hip arthroplasty can get excellent clinical effect, and the length ofincision depend to the body mass index.
