摘　　要：目的:探讨局灶性肺炎的640层螺旋CT表现。方法:回顾性分析了28例经病理证实的局灶性肺炎的CT表现,所有病例胸片均拟诊为周围型肺癌。结果:28例病灶均位于肺野周边部,病变以宽基底与胸膜相连18例,病变边缘呈锯齿状或有粗长毛刺22例,具有周围血管充血聚拢18例,周围存在卫星病灶10例,有胸膜凹陷征8例,6例发现空气支气管征。结论:MSCT检查对局灶性肺炎的诊断及鉴别诊断具有重要价值。Objective： To determine value of 640-slice spiral CT for diagnosis of focal pneumonia. Methods： 28 patients with histologically proved focal pneumonia were studied with MSCT, chest radiolographes for doubt of bronchogenic carcinoma. Results： All 28 lesions were located in peripheral lung field. Eleven of 28 lesions had an irregular margin or large coarse spicule. Extensive pleural thickening was found in 18, convergence of vessels in 18. 10 lesions presented satellite lesion. Pleural indentation was found in 8, air bronchogram in 6. Conclusion： Focal pneumonia has some specific radiographic features, However, it is difficult to differentiate focal pneumonia from peripheral carcinoma with MSCT, correct diagnosis must be based on pathology.
