济南铁路局职工高血压发病趋势及人群特征分析

摘　　要：目的 探讨济南铁路局在职职工高血压发病趋势及流行病学特征.方法 对2006年1月至2010年1月济南铁路局60岁以下在职职工每年进行健康查体流行病学调查,筛选出SBP≥140 mm Hg（1 mm Hg=0.133 kPa）和/或DBP≥90 mm Hg或既往有高血压史、目前正服用抗高血压药物的患者,对其年检出率及人群分布特征进行回顾性分析.结果 济南铁路局在职职工高血压病5年检出率分别为[2006年：22.0％ （5530/25 179）、2007年：23.3％ （6124/26 328）、2008年：28.1％（ 7652/27 194）、2009年：30.7％（7366/24008）、2010年：31.2％（3750/12004）],呈逐年上升趋势（Z=20.72,P=0.001）；其中男性检出率[25.4％（5090/20070）、27.0％（5554/20 576）、32.1％ （6888/21 464）、34.1％（6631/19 430）、34.8％ （3382/9714）]大于女性[8.6％ （440/5109）、10.0％（570/5752）、13.3％（ 764/5730）、16.0％（ 735/4578）、16.1％（368/2290）].随着年龄的增长高血压检出率呈增高趋势；高血压人群中伴高血糖、脂代谢异常、超重、肥胖、吸烟、饮酒者明显高于血压正常者.结论 济南铁路局在职职工高血压发病率呈逐年上升趋势,应予以关注,综合评估,全面干预.Objective To explore the incidence and epidemiological features of hypertension staff in Jinan Railway Administration. Methods Form 2006 to 2010, the staff under 60 years of Jinan Railway Administration received annual health checkup. Based on computer screening, all the people with systolic blood pressure（SBP） ~〉 140 mm Hg and / or diastolic blood pressure（DBP） I〉90 mm Hg or previous history of hypertension or currently tak- ing antihypertensive drugs were investigated. The annual detection rate and population distribution characteristic were analyzed retrospectively. Results For the staff, the annual detection rate of hypertension in five years showed an increasing trend（ Z = 20. 72, P = 0. 001 ） [ 2006 ： 22. 0% （ 5530/25179 ）, 2007 ： 23.3% （ 6124/26328 ）, 2008 ： 28.1% （7652/27194）, 2009：30. 7% （7366/24008）, 2010：31.2% （3750/12004） ]. The annual detection rate of hypertension in male was ~25.4% （5090/20070）, 27.0% （5554/20576）, 32. 1% （6888/21464）, 34. 1% （6631/ 19430）, 34. 8% （3382/9714） I, more than that in female 1 8.6% （440/5109）, 10. 0% （570/5752）, 13.3% （764/5730）, 16. 0% （735/4578）, 16. 1% （368/2290）]. The detection rate of hypertension increased with age. The incidence of high blood sugar, lipid metabolism abnormal, overweight, obesity, smoking or alcohol consumption in hypertensive population was significantly higher than in normal blood pressure population. Conclusion The incidence of hypertension in the staff of Jinan Railway Administration is increasing year by year.
