乙状窦后锁孔入路内镜辅助显微镜微血管减压治疗面肌痉挛的临床研究

摘　　要：目的:探讨内镜辅助下的微血管减压术治疗面肌痉挛的手术方法及效果。方法:内镜辅助下运用面肌痉挛微血管减压术探查面神经并分离责任血管,在微血管与面神经间垫入Teflon涤纶绵。结果:术后面肌痉挛立即消失79例,15例面部仍有不同程度抽搐的患者症状在术后1～22周内消失,2例随访至今患侧面部仍有轻度抽搐,无永久性并发症。结论:微血管减压术,疗效确切,使面神经功能可以得到保护,是首选治疗方法;通过内镜辅助治疗可提高微血管减压术的疗效。Objective： To investigate the microvascular decompression method which is assisted with endoscope and the effect in treatment of hemifacial spasm. Methods： Microvaseular decompression with the assistance of endoscope was used to explore the facial nerve and the offending vessels, then they were separated by Teflon. Results：79 cases were cured. There was no permanence complication. 15 cases of facial spasm patients have different degree of symptoms in the postoperative 1 to 22 weeks within disappeared, 2 cases were followed up until now is still mild ipsilateral facial twitch. Conclusion ： Microvascular decompression is an etiotropic treatment of hemifacial spasm. It may be the first choice for its ef- fectiveness and protection of facial nerve function. The curative effect can be improve by the assistance of endoscope.
