背向散射积分技术在原发性高血压左室和主动脉重构的应用价值

摘　　要：目的:探讨背向散射积分技术在原发性高血压患者左室心肌与主动脉重构的应用价值。方法:67例原发性高血压(EH)患者与30例正常者,分别测定室间隔(IVS)心肌和主动脉根部(AO)背向散射积分(IBS)、IBS周期变异幅度(CVIB),左心室质量指数(LVMI),舒张末期IVS厚度(IVSTd)和主动脉根部(AOD)径线。结果:(1)EH组中,IVS和AO的IBS高于正常组(P<0.01～0.05),CVIB低于正常对照组(P<0.05);LVMI、LAD和AOD均高于正常组(P<0.01～0.05);(2)EH组内LVMI与IVS-IBS和AO-IBS呈正相关(相关系数分别为0.323和0.353,P<0.05);LVMI、IVSTd与AO-CVIB呈负相关(相关系数分别为-0.301和-0.465,P<0.05),IVS-IBS与AO-IBS呈正相关(相关系数为0.438,P<0.05)。结论:高血压导致左室重构的同时伴有主动脉重构的发生,背向散射积分技术为定量评价高血压心肌和主动脉损害程度提供了重要参考指标。Objective： Study the application value of myocardial integrated backscatter in the diagnosis of the left ventricular and aorta remodeling in essential hypertensive disease. Methods： Myocardial integrated backscatter （IBS） , cy- clic variation of IBS （ CVIB ） of interventricular septum （ IVS-IBS and IVS-CVIB ） and aortic wall （ AO-IBS and AO- CVIB） , the dimension of aortic root （AOD） and left ventrieular mass index （LVMI） , and interventricular septum thick- ness （IVST） were measured respectively by echocardiography in 67 essential hypertensive （EH） patients and 30 control subjects （Nor）. Results： （ 1 ） IVS-IBS, AO-IBS, LVMI , LAD, AOD and IVST were higher and IVS-CVIB, AO-CVIB, were lower in EH group than in control group （ P 〈 0. O1 - 0.05 ） ; （ 2 ） There were positive correlations between LVMI and IVS-IBS, A.O-IBS, respectively （ r = 0. 323 ＆ 0. 353 ,P 〈 0.01 - 0. 05 ）, and IVS-IBS and AO-IBS （ r = 0. 438 ,P 〈 0.05 ） in EH group. There were negative correlations between LVMI, IVST and AO-CVIB （ r = - 0. 301 ＆ - 0. 465, P 〈 0. 01 0. 05 ）, respectively in EH group. Conclusion：Left ventricular remodeling was accompanied with aortic remodeliag in es- sential hypertension. The change myocardial integrated backscatter serve as an important parameter for estimate the dama- ging degree of the left ventricular and aorta in essential hypertensive disease.
