摘　　要：目的探讨乳头Paget病的病理改变及免疫组化特点。方法收集17例乳头Paget病的临床资料,观察镜下病理及免疫组化特点。结果本组合并乳头下方浸润性导管癌15例,占88.24%;合并乳头下方导管内癌(DCIS)2例,占11.76%。镜下观察:乳头表皮内均见Paget细胞;免疫组化:Paget细胞与其下方的DCIS细胞有相似的特征,表现为雌激素受体(ER)、孕激素受体(PR)低表达,cerbB-2过表达,细胞角蛋白7(CK7)和细胞角蛋白8(CK8)均表达。结论乳头Paget病是一种特殊类型的乳腺癌,CK7、CK8的表达可协助诊断。Objective To explore the pathological changes and immunohistochemistrical characteristics of Paget Disease of the Nipple. Methods Clinical data of 17 patients with Paget Disease of the Nipple were collected for studying the pathological and immunohistochemisty features. Results The patients complicated with invasive cancer or ductal carcinoma in situ （DCIS） accounted for 88.24% （15/17） and 11.76% （2/17） , respectively. Paget cells were observed in the epithelial cells under a microscope for invasive cancer and DCIS. Immunohistochemistry showed that Paget cells and the underlying intraductal carcinoma cells had similar features, including low expression of ER and PR, overexpression of cerbB-2 and expression of CK7 and CK8. Conclusion Paget Disease of the Nipple is a special type of breast cancer. Expression of CK7 and CK8 can assist the diagnosis.
