摘 要:目的应用超声斑点追踪成像(STI)技术,测量正常人左室短轴收缩期径向应变(RS)峰值。方法 50例健康人,应用超声心动图诊断仪获得左室短轴二尖瓣、乳头肌及心尖3个水平高频二维动态图像,使用Qlab8.1工作站进行脱机分析,将左室短轴3个水平心肌等分为心内膜下层、心外膜下层及整体心肌为感兴趣区,应用超声STI分析感兴趣区收缩期RS峰值。结果①正常人左室短轴心内膜下层心肌各节段收缩期RS峰值均高于心外膜下层各节段(P<0.05),各水平间各节段收缩期RS峰值相比,二尖瓣和乳头肌水平均>心尖水平(P<0.05),二尖瓣及乳头肌水平间差异无统计学意义。②正常人左室短轴各水平间各节段整体心肌收缩期RS峰值相比,乳头肌>二尖瓣>心尖水平(P<0.05)。③正常人左室短轴各水平间各节段心外膜下层心肌收缩期RS峰值相比,差异无统计学意义。结论超声STI不受声束角度的限制,可以准确地评价左室心肌各节段收缩期RS,为测定局部和整体心肌收缩功能以及心肌疾病的定位诊断提供新方法。Objective To evaluate left ventricular(LV)systolic radial strain(RS)by echocardiography Speckle Tracking imaging(STI)in health subjects.Methods It was obtained high frame rate two-dimensional images by echocardiography device for 50 healthy subjects and analysis based on Qlab8.1 work station.The system automatically divided three level myocardium(mitral valve,papillary muscles and apex)of LV short-axis into subendocadium,subepicadium and whole wall for the region of interest,and then the peak values of systolic RS in the region of interest were measured by two-dimensional ultrasound STI.Results The radial strain of the left ventricular short-axis of the inner layers showed higher than those of the outer layers in healthy subjects(P〈0.05).The peak systolic radial strain of the subendocadium of the left ventricular short-axis views in healthy subjects was the levels of last apexand apex.The peak systolic radial strain of the left ventricular short-axis views in health subjects were the level of papillary muscle 〉mitri annulus〉apexand apex.The peak systolic radial strain of the subendocadium of different left ventricular short-axis views had no statistical difference.Conclusion Ultrasound STI is independent of insonation angle for assessing myocardial RS.It is of the potential to be used clinically in evaluation of LV regional and whole myocardial systolic function is a new method in diagnosing myocardial diseases.
