等离子电切术在宫腔疾病中的应用观察

摘　　要：目的探讨和评价等离子双极电切宫腔镜在宫腔疾病中应用的临床有效性和安全性。方法应用英国佳乐等离子双极电切宫腔镜对行子宫内膜息肉电切术3102例患者进行治疗。结果所有患者手术经过顺利,出血量(40±30)ml,灌注生理盐水500～9000 ml;切除组织2～60g,平均15g;住院4～10 d,切除标本均病检,手术效果良好。1例宫腔息肉合并纳氏囊肿切除囊肿时3、9点部位出血,0号可吸收线子于宫颈3、9点各"8"字缝合1针止血;术后宫颈管黏连4例,行宫颈管扩张治愈。无其他并发症。结论等离子双极电切宫腔镜镜技术安全、有效,术中出血少,视野清楚,并发症少。Objective To investigate and evaluate the clinical efficacy and safety of the dipolar plasmakinetic resection hysteroscopy in the patients with uterine diseases.Methods Application of the Britain Gerys dipolar plasma kentic resection hysteroscopy,the age of patients were 23 years-65 years.Resection of endometrial polyp in 3102 cases,included the Nessler’s cyst in 40 cases,while made the Nessler cyst resection;resection of submucosal fibroid in 75 cases;Resection of uterine septum in 3 cases.The operation time was 5 minutes^50 minutes,average 20minutes.Results The surgery in all patients were successfully.Blood loss was 40 ± 30ml,infusion of saline was 500 ml^9 000 ml,removal of tissue was 2g^60g,average 15g,4 days^10 days in hospital.Resection specimens need pathological examination,all surgery with good results.One case of uterine polyps combined cyst Nessler with 3 o’clock and 9 o’clock site bleeding,sutures the 3 o’clock and 9 o’clock site of cervix with absorbable suture to stop bleeding,4 cases with cervical adhesions were cured by cervical tube expansion after operation and there were no other complications.Conclusion Dipolar plasmakinetic resection hysteroscopy are efficacy and safety,with less bleeding,clear vision,and fewer complications.
