摘 要:目的:研究盐酸右美托咪定用于双腔气管插管和术后拔管期血流动力学的影响,以及对苏醒时间的影响。方法:选择肺部短小手术患者60例按随机数字表分为两组:实验组(Ⅰ组)和对照组(Ⅱ组)各30例。实验组泵入盐酸右美托咪定直至手术结束。对照组持续泵入丙泊酚4~8mg/kg.h直至手术结束。记录患者插管及拔管期间的基础生命体征,并记录苏醒时间、拔管时间及病人清醒后的耐管情况和吸痰反应。结果:气管插管期间,两组患者的血压、心率均有显著变化,Ⅱ组变化更显著;气管拔管期间,与Ⅱ组比较,Ⅰ组患者在苏醒后、拔双腔管前、拔双腔管后血压、心率明显降低;Ⅰ组术后清醒时间、拔管时间明显短于Ⅱ组,Ⅰ组病人清醒后的耐管效果好于Ⅱ组,吸痰反应Ⅰ组轻于Ⅱ组。结论:右美托咪定用于胸腔镜下肺大疱切除术中可减轻双腔管置入时的血流动力学变化,能明显增强患者对双腔管的耐受性,减轻吸痰反应,缩短苏醒时间和拔管时间。Objective: In this article, dexmedetomidine hydrochloride are given (Dexmedetomidine hydrochloride, DMED) for double-lumen endotracheal intubation and after extubation of the hemodynamie effects and the effects of the recovery time. Methods:We select 60 patients who going short surgical lung were randomly divided into 2 groups(n= 30 each) :experiment group ( I ) and control group ( II ). Experiment group: We pump continuously dexmedetomidine to 0. 2-0. 9ug/(kg. h) until the end of surgery. Control group: We pump continuously propofol to 4-8mg/kg. h until the end of surgery. We record basic life indications during the intubation and the extubation, the recovery time, the resistance situation and the suction tube reaction. P〈0.05 was considered statistically significant. Results:During the intubation period, SBP, DBP and HR in all patients rise obviously when double-lumen tube inserted. II group change more significant;During the extubation , differentiation about SBP,DBPand HR exsists between the experiment group and the control group in after waking, before pulling double-lumen tube and after pulling double-lumen tube;About the postoperative recovery time and extubation time, group I was shorter than group II. Resistant tube after waking in group I is better than in group II Suction reaction is lighter in group I than in II group. Conclusion: Dexmedetomidine hydrochloride scheduled for thoracoscopic lung resection of bullae not only can reduce the Hemodynamic changes when double-lumen tube is inserted, en- hance the patient's tolerance of double-lumen tube, reduce the suction reaction hut also can shorten the recovery time and extubation time.
