“强心Ⅱ号”改善肺源性心脏病心力衰竭患者血气及生活质量

摘　　要：目的 :探讨中药“强心 号”治疗肺源性心脏病心力衰竭 (肺心病心衰 )的疗效及患者血气和生活质量的变化。方法 :98例患者随机分为 2组 ,对照组 (41例 )以肺心病心衰常规西药治疗 ;治疗组 (5 7例 )在对照组治疗的基础上 ,加服中药“强心 号”,每日 1剂 ,2组均治疗 2周。分别测定 2组治疗前后动脉血氧分压 (Pa O2 ) ,二氧化碳分压 (Pa CO2 ) ,血氧饱和度 (Sa O2 )及 p H值 ,且按 Barthel指数评定 2组患者生活质量的变化。结果 :治疗组的显效率及总有效率 (73.7%和 96 .5 % )分别优于对照组 (31.7%和 73.2 % ) ,均有显著性差异 (P<0 .0 1和 P<0 .0 5 ) ,其改善 Pa O2 、Sa O2 及 Pa CO2 较对照组显著 (P<0 .0 1和 P<0 .0 5 ) ,纠正心衰平均时间显著缩短(P<0 .0 5 ) ,并可提高患者生活质量。结论 :“强心 号”治疗肺心病心衰疗效肯定 ,并可减少利尿剂和洋地黄类强心西药的不良反应。Objective:To observe the effects of traditional Chinese medicine cardiotonicⅡ(强心Ⅱ号) of heart failure caused by pulmonary heart disease,and the improvement of blood gas and life quality in the patients.Methods:98 patients were randomly divided into two groups.The control group (41 cases) was treated with western medicine,and treatment group (57 cases) was treated by adding one dose of traditional Chinese medicine cardiotonicⅡ every day on the base of western medicine therapy.The therapeutic course was two weeks for both groups.The PaO 2,PaCO 2,SaO 2 and pH in arterial blood of the patients pretreatment and posttreatment respectively were determined,and the improvement of patients′ life quality by Barthel Index were evaluated.Results:The markedly effective rate and total effective rate (73 7% and 96 5%) in the treatment group were higher than those in control group (31 7% and 73 2%) respectively (P<0 01 and P<0 05).The respect of improving PaO 2,PaCO 2 and SaO 2 in the treatment group was better than those in control group significantly (P<0 01 and P <0 05),as well as shorting the average time of correcting heart failure ( P <0 05).The life quality of patients in treatment group was improved.Conclusions:The treatment for heart failure caused by pulmonary heart disease with cardiotonicⅡ is effective,and it can reduce the untoward effect of western medicine as diuretic and digtalis cardiotonic.
