非胰岛素依赖型糖尿病患者视网膜病变与24小时尿白蛋白排泄率的相关性

摘　　要：目的 :观察 NIDDM患者视网膜病变 (DR)与 2 4h尿白蛋白排泄率 (UAER)的相关性。方法 :对 138例 NIDDM患者同时进行眼底荧光血管造影 (FFA)及放射免疫法测定 UAER。结果 :糖尿病视网膜病变的发生和严重程度随 UAER增加而加重 ;正常、背景型和增殖型视网膜病变 UAER平均值分别为 9.97± 9.8μg/min、 2 7.0 9± 32 .5 9μg/m in和 6 2 .70±47.6 8μg/m in (P <0 .0 5 )。同样 ,白蛋白尿发生率也随 DR程度的加重而显著升高。结论 :NIDDM患者 DR和 UAER显著相关 ,同时进行 FFA及 U AER的检测 ,对糖尿病肾病的早期诊断有重要价值。Objective:To observe the correlativity between the urinary albumin excretion and retinopathy in patients with non insulin dependent diabetes mellitus(NIDDM) Methods:24 hour urinary albumin excretion was measured by specific radioimmunoassay and fluorescein fundus angiograph(FFA) was studied in 138 patients with NIDDM Results:the prevalence and degree of diabetic retinopathy(DR) increased significantly with increase of urinary albumin excretion The average rates of the normality,the background type and proliferative type with UAER were 9 97±9 8μg/min、27 09±32 59μg/min and 62 70±47 68μg/min(P<0 05),respectively UAER increases as DR becomes severe Conclusion:our data suggest that DR in NIDDM patients is associated significantly with urinary albumin excretion This investigation suggests that both FAA and UAE are useful in the diagnosis of diabetes nephropathy
