参麦注射液治疗冠状动脉旁路移植术后合并左心功能不全39例临床观察

摘　　要：目的观察参麦注射液对冠状动脉旁路移植术(CABG)后合并左心功能不全患者的影响。方法将78例CABG术后合并左心功能不全患者随机分为2组,对照组39例予西医常规治疗,治疗组39例在对照组治疗基础上加参麦注射液治疗。2组均14 d为1个疗程,共治疗1个疗程。观察2组治疗前后收缩压、舒张压、心率及左室收缩功能指标[包括左室舒张末期内径(LVEDD)、左心房内径(LAD)、心排血量(CO)、左室射血分数(LVEF)、二尖瓣舒张早期血流速度峰值(E峰)、舒张晚期血流速度峰值(A峰)及E/A值]及心功能变化情况,统计不良反应发生情况。结果 2组治疗后收缩压、舒张压及心率与本组治疗前比较均无明显变化(P>0.05),且2组治疗后组间比较差异均无统计学意义(P>0.05)。2组治疗后LVEDD、LAD、CO、LVEF、E峰、A峰、E/A及心功能均较本组治疗前明显改善(P<0.05),且2组治疗后组间比较差异均有统计学意义(P<0.05),治疗组改善优于对照组。2组均无明显不良反应。结论参麦注射液可明显改善CABG术后合并左心功能不全患者心功能,疗效确切。Objective To observe the effect of Shenmai injection on the treatment of coronary artery bypass graft （CABG） with left ventricular dysfunction. Methods 78 patients with coronary artery bypass graft with left ven- trieular dysfunction were randomly divided into two groups. Patients in control group（ n = 39） were treated by routine western medicine therapy. 39 cases in treatment group were additionally treated by Shenmai injection on the basis of routine therapy. The therapeutic course was 14 days in two groups. Systolic pressure, diastolic pressure, heart rate and left ventricular systolic function（ LVEDD, LAD, cardiac output, LVEF, early and late diastolic blood peak velocity （ E, A） of mitral valve and E/A ratio） and changes of cardiac function before and after treatment were observed aftera course. Results There was no significant difference between before and after treatment on systolic pressure, dias- tolic pressure and heart rate in two groups（ P 〉 0.05 ）. And there was no significant difference between two groups af- ter treatment（ P 〉 0.05 ）. LVEDD, LAD, cardiac output, LVEF, E and E/A ratio after treatment were increased as compared with those before treatment in two groups（ P 〈 0.05 ）, and A was decreased（ P 〈 O. 05 ）. Cardiac function were significantly improved （ P 〈 0.05 ）. There was significant difference between two groups after treatment （ P 〈 0.05 ）. The improvement of treatment group was better than that in the control group （ P 〈 0.05 ）. Both groups were without obvious adverse reactions. Conclusion Shenmai injection has definite effect and can obviously improve Car- diac function on the treatment of coronary artery bypass graft with left ventricular dysfunction
