摘　　要：目的　了解股骨大转子股骨头互换 (TEFH)股骨头重建疗效。方法　用自身带血供股骨大转子作人工股骨头 ,修复重建中青年患者股骨头颈毁损。结果　解剖测量发现 ,股骨大转子可完整地替代股骨头。动物实验证实 ,新股骨头成活 ,并逐渐向股骨头转化 ,为临床应用奠定了物质基础。临床设计应用了 5种大转子股骨头互换术 ,治疗中青年股骨头颈毁损72例 ,随访 1～ 8年 ,疗效满意。结论　TEFH具有人工髋关节修复重建能力和功能 ,避免了人工股骨头假体的缺点及并发症。? Objective To determine the effects of the reconstructed femoral head by the femoral great trochanter exchanging with femoral head(TEFH).Methods Ones own vascularized femoral great trochanter was used as the femoral head component to reconstruct the damaged femoral head of the young patients. Results After measuring femoral samples,we found that the femoral great trochanter can replace the whole femoral head.The animal experiment results confirmed that the reconstructed femoral heads were all viable,and it turned towards normal femoral heads progressively,which laid the foundation for longer usage.Five patterns of TEFH have been developed clinically,and 72 cases were treated with this technique,all of whom were young patients with severe damaged femoral heads and necks.With 1 to 8 years follow up,the results were satisfactory.Conclusions The TEFH owns the reconstructional abilities and functional effects of artificial hip joint,while it overcomed the short lived defect of the latter.It provides an ideal method to treat young patients with severe damaged femoral head and neck. 〔
