痛经宁对气滞血瘀型原发性痛经患者血清前列腺素和β-内啡肽的影响

摘　　要：目的观察痛经宁对气滞血瘀型原发性痛经的临床疗效及对血清前列腺素、β-内啡肽的影响。方法将70例气滞血瘀型原发性痛经患者随机分成治疗组和对照组各35例。治疗组服用自拟痛经宁汤剂,对照组服用痛经宝颗粒。观察两组临床疗效和治疗前后血清前列腺素E2(prostaglandin E2,PGE2)、血清前列腺素F2α(prostaglandin F2α,PGF2α)、β-内啡肽(β-endophin,β-EP)水平的变化。结果治疗组、对照组分别脱落4例和5例。治疗组愈显率显著高于对照组(P<0.05)。与正常组比较,治疗前两组PGE2、PGF2α、β-EP差异均有统计学意义(P<0.05);与治疗前比较,治疗后治疗组3种指标的血清水平均有显著变化(P<0.05),对照组仅PGF2α和β-EP的血清水平发生显著变化(P<0.05)。结论痛经宁治疗气滞血瘀型原发性痛经具有较好的临床疗效,且对前列腺素、β-内啡肽有调整作用。Objective To observe the clinical efficacy of Tongjingning, a traditional Chinese medicine, in the treatment of primary dysmenorrhea （PD） of qi stagnation and blood stasis type, and observe its effects on serum levels of prostaglandin and β-endophin （β-EP）. Methods Seventy patients with PD of qi stagnation and blood stasis type were averagely randomized into a treatment group and a control group. Patients in the treatment group were treated with Tongjingning Decoction, while patients in the control group were treated with Tongjingbao Grannules, a patent traditional Chinese medicine. The ciinical efficacy was observed, and the changes of serum levels of prostaglandin E2 （PGE2）, prostaglandin F2a （PGF2a） , and β-EP were assayed before and after treatment. Results There were 4 and 5 patients lost to follow up in the treatment group and the control group, respectively. The cure and marked effectiveness rate in the treatment group was sig- nificantly higher than that in the control group（P（0.05）. Before treatment, the serum levels of PGE2 , PGF2a, and β-EP in the treatment group and the control group were significant different from those in the normal group （P〈0.05）. After treatment, the serum levels of the three indexes were significantly differ- ent from those before treatment（P〈0.05） , while only two indexes, including PGF2o and β-EP, had sig- nificant changes in the control group（P〈0.05）. Conclusion Tongjingning has good effects in treating PD of qi stagnation and blood stasis type, and it can adjust the serum levels of prostaglandins and β-endorphin.
