高明利教授辨证治疗皮肌炎和多发性肌炎

摘　　要：高明利教授认为皮肌炎和多发性肌炎属中医痿、痹证范畴,多为感受外邪,湿、热、毒夹杂,导致阴阳气血失调,脏腑受损。分三期论治,初期祛邪为主,湿热蕴结则以清热祛湿、解肌通络,予四妙散、当归拈痛汤;毒热炽盛治以清热解毒、凉血通络,予犀角地黄汤加减。中期扶正祛邪并施,以健脾祛湿,化痰通络为法,予香砂六君子汤、茯苓散合控涎散加减。缓解期多虚,注意顾护胃气,不可攻伐太过,治疗以益气养血,透热养阴为法,予补中益气汤、青蒿鳖甲汤和三痹汤加减;肢体偏瘫者予补阳还五汤加减。Professor Gad Mingli consider that the dermatomyositis and polymyositis is included in Category of flaccidity and arthralgia syndrome in traditional Chinese medicine . The ill is invaded by external evil including of wet,heat and toxicity,result in disorder of Yin,Yang, Qi, blood and the viscera. Clinical treatment is divided into three stage. In the early,as Damp -heat stagnate syndrome, professor Gad Mingli apply Simiao Powder and Dangguiniantong Tang to treat this disease, throught the principle of clearing - heat, taking away the damp, unlocking skin and passing meridian. As Toxic hot flaming syndrome, he apply addition and subtrac- tion of Xijiaodihuang Tang to treat this disease throught the principle of clearing - heat, detoxicating, cooling blood and dredging the main and collateral channels. In the Medium - term, to support healthy atmosphere and remove evil, Professor Gad Mingli advocate that it is used addition and subtraction of XiangshaLiujunzi Tang, Fulin San and Kunyan San, taking away the wet, removing sputum and passing meridian throught improving Spleen,. In the late period, Professor Gad think that pay attention to protect qi of stomach , not attack too much. For treatment, he comply addition and subtraction of Buzhongyqi Tang, Qinghaobiejia Tang, and Sanbi Tang to treat, throught the method of invigorating qi, nourishing blood, taking away heat, and nourishing Yin. If the patient has limbs paralysis, professor Gad applyes addition and subtraction of Buyanghuanwu Tang.
