加味当归四逆汤联合西药治疗糖尿病周围神经病变对照观察

摘　　要：[目的]观察加味当归四逆汤联合西药治疗糖尿病周围神经病变疗效。[方法]将68例住院及门诊患者,按随机数字表法分为两组。均常规降糖、饮食控制血糖在正常范围。对照组33例维生素B1100mg,维生素B6100mg,肌肉注射,1次/d。治疗组35例,在对照组基础上加用加味当归四逆汤(当归15g,桂枝6g,细辛3g,白芍10g,通草6g,川芎5g,干姜6g,大枣15g,甘草6g)水煎服,1剂/d,分2次服。观察腱反射、膝反射,自觉证状;腓总神经、胫神经、腓浅神经传导速度;血液流变学指标;血、尿、便三大常规及肝肾功能,毒副反应。均连续治疗15d为1疗程。治疗1疗程(15d)判定疗效。[结果]综合疗效治疗组优于对照组;肌神经传导速度两组均有明显提高,治疗组优于对照组;血液流变学指标两组均有明显改善,治疗组优于对照组;未发现有毒副作用。[结论]加味当归四逆汤联合西药治疗糖尿病周围神经病变优于单纯西药治疗,能改善降低血液流变学指标,改善血管内皮功能及神经营养,未见明显毒副反应,安全、有效,有临床推广及科学研究价值。[ Objective ] Danggui Sini Tang combined with Western medicine treatment of diabetic peripheral neuropathy. [ Method]68 cases of inpatient and outpatient, divided into two groups by random number table. Both conventional hypoglycemic diet to control blood sugar in normal range. The control group of 33 cases vitamin B1l00mg, vitamin B6100mg, and intramuscular injection, 1 time/day. Treatment group 35 cases, on the basis of the control group Danggui Sini Tang, In （ Danggui 15g, Guizhi 6g, Xixin 3g Baishao 10g, Tongcao 6g, Cuanxiong 5g, Ganjiang 6g, Dazao 15g, Gancao 6g） decoction, one / day, twice serving. Tendon reflex, patellar reflex, conscious card - like ; peroneal nerve, tibial nerve, superficial peroneal nerve conduction velocity ; hemorheology; blood, urine, will be three practices and liver and kidney function, and toxicity were observed. Are continuous treatment of 15 days as a course of treatment. Treatment course of treatment （ 15 days） to determine efficacy. [ Results ] The efficacy of the treatment group than the control group ; muscle nerve conduction velocity in both groups improved significantly, the treatment group than the control group ; hemorheology two groups showed significant improvement in the treatment group than the control group ; not The discovery of toxic side effects. [ Conclusion ] Danggui Sini Tang combined with Western medicine treatment of diabetic peripheral neuropathy is better than western medicine alone can improve to reduce the hemorheological indexes to improve vascular endothelial function and neurotrophic, and no significant toxicity, safe, effective, clinical and scientific value.
