新疆某三甲医院脑梗塞患者住院费用影响因素分析

摘　　要：目的 揭示新疆某院脑梗塞患者住院费用的主要影响因素,为单病种费用控制提供参考.方法 收集1 326例脑梗塞病例的有关信息,采用多元逐步回归法对脑梗塞患者住院费用影响因素进行分析.结果 脑梗塞患者住院费中,以药费费用最高,其次是治疗费,依次是检查费、其他、床位费、护理费.脑梗塞患者住院费用经多元逐步回归分析,年龄X2、入院情况X4、住院天数X5、治疗结果X6 对住院费用有影响.从标准回归系数看,住院费用的影响因素由大到小依次为住院天数、入院情况、年龄、治疗结果.住院费用回归方程为：＝3.659 1＋0.002 5X2-0.096 9X4＋0.007 5X5＋0.050 4X6,从方差分析表可知,F=45.69,P〈0.01,故回归方程是有意义的.结论 有效控制脑梗塞患者住院费用应以控制和缩短住院天数为突破口.同时应建立科学的医疗费用审查和评估体系,对医院的医疗行为实行全过程监控.Objective To approach the factors influencing hospitalization expenses of cerebral infarction patients in or- der to control the simple disease costs. Methods The data of 1 326 cases of cerebral infarction patients were collected to ana- lyze the factors influencing hospitalization expenses through multiple stepwise regressions. Results Of all medical expenses, drug costs ranked first in proportion followed by examination, other miscellaneous expenses, bed fee, and nursing fee. The main factors influencing the hospitalization expenses in cerebral infarction patients included the age （X2 ）, patient＇s condition （X4）, LOS （length of stay）（Xs ） and therapeutic outcome（X6 ）. The optimal factor is selected and the regression equation is estab- lished by introduction of multiple factors and stepwise regressions. Y= 3. 659 1 ＋0. 002 5X2 -0. 096 9X4 ＋0. 007 5X5 ＋0. 050 4X6. F = 45.69,P〈0.01. Conclusion In order to control effectively the hospitalization expenses, it was a feasible way tosave money by shortening hospitalization days, meanwhile, a scientific assessment system should be established to supervise the medical prac- tice.
