横截包皮带蒂岛状皮瓣一期成形术治疗尿道下裂(附56例报告)

摘　　要：目的:探讨横截包皮带蒂岛状皮瓣一期成形术治疗尿道下裂的效果,提高手术成功率。方法:采用横截包皮带蒂岛状皮瓣一期成形术治疗尿道下裂56例,其中阴茎头型14例,阴茎体型32例,阴囊型10例。结果:一次性手术成功42例(75%)。手术后出现尿瘘16例(28.6%),其中6例于手术后3月内尿瘘自愈,8例于手术6个月后行尿瘘手术修补治愈。2例尿道裂开,6个月后行尿道板纵切卷管尿道成形术治愈。无1例出现尿道狭窄。结论:横截包皮带蒂岛状皮瓣一期成形术是治疗尿道下裂的一种较好的基本手术方法,术中保证带蒂岛状皮瓣良好的血供和术后精心护理是手术成功的关键所在。Objective：To explore the effection of the transverse pedicled foreskin flaps for congenital hypospadias,and to improve the success rate.Methods： 56 cases of hypospadias were treated by the transverse pedicled foreskin flaps among it,14 cases of penile head hypospadias,32 cases of penile size hypospadias,10 cases of scrotal hypospadias.Results：42 cases were successful by only one surgery.16 cases were urinary fistula after surgery,among it,6 cases self-healed after surgery in three months,8 cases were cured by repair surgery after six months.2 cases of urethral split,they were cured by urethroplasty of tubularized incised plate.No one cases of urethral stricture.Conclusion：The transverse pedicled foreskin flaps is a good basic urethroplasty for hypospadias.The good blood supply of pedicled island flap and intensive care after surgery are the key of successful operation.
