静脉血栓患者凝血功能、纤溶动态变化的研究分析

摘　　要：目的分析静脉血栓患者凝血功能和D-二聚体、FDP的动态变化,探讨静脉血栓患者凝血功能和D-二聚体、FDP变化机制,为临床提供客观的实验室凝血功能数据分析。方法采用日本Sysmex-CA 6000全自动血凝仪对2011年12月～2012年4月我院78例静脉血栓病例不同患病时间与45例正常体检的标本凝血功能和D-二聚体、FDP进行检测分析。结果静脉血栓组凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)均低于正常对照组,有显著性差异(P<0.01);纤维蛋白原(FIB)、D-二聚体(D-D)、FDP含量均高于正常对照组,有显著性差异(P<0.01)。患者D-二聚体值在急性期内含量较高,治疗后20～30d下降至正常水平,差异有统计学意义(P<0.05)。结论静脉血栓患者凝血功能指标和D-二聚体、FDP含量不同时期均存在动态变化,观察静脉血栓患者的凝血功能和D-二聚体、FDP的变化可作为判断患者病情严重程度及其恢复的重要诊断指标,为临床诊治静脉血栓疾病提供客观实验室数据。Objective To provide an objective test method for the clinical laboratory by observing the dynamic changes of the coagulation function, D-dimer （DD） and FDP in patients with venous thrombosis and by exploring the mechanism of the above changes.. Methods 38 patients in hospital with venous thrombosis from December 2011 to April 2012 and 45 cases with normal physical examination results were compared for collecting data about their respective clotting function, DD and FDP. Resuits There were less prothrombin time （PT）, activated partial thromboplastin enzyme time （APTT） and thrombin time （TT） and more fibrinogen （FIB） DD and FDP in the group with venous thrombosis, compared with those in the control group （P 〈 0.01）. The levels of DD in patients in the acute stage were higher and then 20-30 days after treatment reduced to the normal level, which had statistical significance （P 〈 0.05）. Conclusions The data from observations on dynamic changes of the clotting function, DD and FDP could be important laboratory indicators for the clinical diagnosis and treatment of venous thromboembolism diseases.
