雌性激素与胆石成因的实验及临床研究

摘　　要：目的 :探讨雌性激素在胆囊成石过程中的作用。方法 :通过致石性饮食建立胆结石的豚鼠动物模型 ,采用放射免疫法及酶联亲和抗体组织化学法 ,对 32只实验豚鼠和 32例对照组及 45例胆囊结石组患者分别进行了血清雌二醇 (E2 )、孕激素 (Pg)浓度及肝脏、胆囊组织雌激素受体 (ER)、孕素受体 (PR)的检测。结果 :实验组豚鼠胆结石的发病率为 84% (13/ 15 ) ,明显高于对照组 (P〉0 .0 5 ) ,且实验组豚鼠成石均匀为以胆固醇类为主的结石 ,其血清 E2 浓度与肝脏 ER、PR阳性表达均高于对照组 (P〈0 .0 1) ,但其肝脏 ER阳性率与非结石患者无明显差别 (P〉0 .0 5 ) ,所有结石组胆囊组织 ER及 PR阳性表达均高于相应各期对照组 (P〈0 .0 5 )。结论 :血清孕激素含量增高可能与育龄期女性的胆囊结石发病有关 ,而血清雌激素 (E)含量增高可能与育龄期和绝经后女性的胆囊结石发病有关。揭示 E、Pg通过它们相应的受体增加胆囊胆固醇结石的易感性 ,提高胆囊结石发病率。Objective:To determine the role of estrogen(E 2)in the development of gallstone formation.Methods:Animal models of cholesterol gallstone induced by cholesterolenriched diet were established on guinea pips.The serum estrogen and progesterone level,also estrogen and progesterone receptors in the liver and gallbladder were evaluated respectively by using radioimmunoassay(RIA)and PAP immunohistochemical method in 32 guinea pigs of the animal exper imental group,32 patients of normal controls and 45 patients with gallstones.Resuls The incidences of gallstone in guinea pigs are 84 percent (P〈0.05),The experimental animals suffered with cholesterol gallstone.The difference and plasma estrogen concentrations and the positive expressions of estrogen and progesterone receptor in the liver between two naimal groups was significantly(P〈0.01). So like is significantly about the serum estrogen level of female gallstone patients(P〈0.01)But the diffirence of the positive expressions of estrogen receptor in the liver between two groups are not significantly(P〉0.05).The positive expression of estrogen and progestrone receptor in the gallbladder of all of the female gallstone patients between two groups are significantly(P〈0.05).Conclusion:The increase of serum estrogen and progestrone level might play an important role in the devel also play an important role after menopacdse.It's suggested that estrogen and progestrone inhance the susceptibility of patients to the cholesterol gallstone through their corresponding recept ors expression in the liver and gallbladder.
