摘  要:目的探讨中重度新生儿缺氧缺血性脑病(HIE)的影像诊断及应用复方丹参联合神经节苷脂治疗的临床疗效。方法 2009年1月~2011年2月间入住本院的中重度HIE患儿共96例,依据时间阶段先后分为两组,即2009年1月~2010年1月间收治的46例为对照组,2010年2月~2011年2月间收治的50例为治疗组。在常规治疗的基础上,对照组给予胞二磷胆碱125ml/天,或脑活素5ml/天,治疗组给予复方丹参4ml/天及神经节苷脂20mg/天,共治疗3~5个疗程,并随访至生后6个月,治疗过程中行头颅CT或MRI诊断及动态观察。结果治疗组在生后10~14天,28天NB-NA评分,28天头颅CT或MRI恢复情况明显好于对照组(t=2.91,P<0.05;t=3.99,P<0.05;χ2=4.51,P<0.05),6个月时后遗症的发生率治疗组较对照组明显降低(χ2=3.89,P<0.05),两组比较有显著性差异。结论头颅CT或MRI是对HIE诊断和疗效观察的良好方法,复方丹参联合神经节苷脂治疗中重度HIE在早期临床症状恢复及新生儿期后预后改善方面明显好于胞二磷胆碱或脑活素组。Objective To evaluate imaging diagnosis and clinical efficacy of combined composite salvia miltiorrhiza bunge and ganglioside on moderate-severe degree hypoxic-ischemic encephalopathy (HIE) of newborn. Methods From Jan 2009 to Feb 2011. A total of 96 newborn diagnosed as HIE by clinical feature and CT/MRI findings were divided into the treat- ment group (50 cases) and the control group (46 cases). Onthe foundation of conventional treatment, the treatment group received composite salvia miltiorrhiza Bunge 4 ml/d and ganglioside 20 rag/d, the control group received citicoline 125ml/ d or Cerebrolysin 5 ml/d. A total of 3-5 courses of treatment were performed with follow-up to 6 months after birth. During the treatment, CT or MRI were performed for dynamic observation. Results At l0b 14 d, 28 d. the NBNA score of treatment group was higher than that of control group; CT or MRI findings of treatment group at 28 d were better than control group. (t =2.9i, P〈0.05; t =3.99, P〈0.05;χ^2=4.51, P〈0.05). The sequela incidence of treatment group was lower than control group (χ^2 = 3.89, P〈0.05). Conclusion Brain CT or MRI was an effective way to diagnose and evaluate efficacy of HIE. Combined composite salvia miltiorrhiza Bunge and ganglioside on moderate-severe degree HIE of newborn was better than the group of using citicoline or cerebrolysin.
