摘 要:[目的]旨在建立与人肩关节骨-肌腱结合部损伤相似的动物模型。[方法]通过在SD大鼠肩关节冈上肌与肱骨骨腱结合部造成损伤与重建手术,分别于术后7、14、21 d处死大鼠,切取冈上肌腱远端以及与其相连的肱骨骨块为一个完整的BTJ样本,并取未手术肩袖冈上肌的正常BTJ样本为对照,并通过HE染色,观察比较愈合过程不同时间BTJ中瘢痕组织的形成和转归特点及纤维软骨的分布及其与正常BTJ的差别。用免疫组织化学方法比较不同时间BTJ组织内TGF-β1、AR-Smads(包括Smad 2和3)的分布及其与正常BTJ的差别。[结果]HE染色肌腱纤维、软骨以及骨组织层次清楚,细胞排列整齐。术后7、14 d梭形成纤维细胞满视野。术后21 d成纤维细胞内可见散在少量软骨细胞。免疫组化结果:正常对照组的TGF-β1、AR-Smads呈低表达;术后7、14、21 d时TGF-β1和AR-Smads表达明显高于对照组,且均在14 d时达到高峰。[结论]该大鼠肩关节骨腱结合部损伤动物模型的建立较好的模拟了BTJ损伤后的愈合过程,是一种新型、可靠、理想的动物模型。[ Objective ] To establish an injury animal model similar to human shoulder bone-tendon junction ( BTJ ). [ Method] ASD rats model in shoulder supraspinatus muscle tendon junction was made through the humerus bone damage and reconstructive surgery. Rats were killed respectively at 7,14,21 d after surgeries ,and were cut at supraspinatus distal tendon and the connecting humerus bone to form complete BTJ samples. After taking the normal non-surgical BTJ control sample of shoulder rotator cuff supraspinous muscle, we performed HE staining, observing and comparing at different time points by BTJ healing process on the basis of scar tissue fomaation and turnover characteristics, together with the distribution of fibrous cartilage and difference with normal BTJ. We compared the immunohistochemical distribution results by the measurement of TGF-~I ,AR- Smads ( Smad2 and Smad3 ) between BTJ samples and normal BTJ. [ Result ] Clear layers of muscle tendon fibers, cartilage, and hone tissues were confirmed. Cells were arranged trimly. Mass collagenoblasts could be seen in microscopic view. Small amounts of cartilage cells were seen 21 days postoperatively. Immunohistochemical results showed TGF-[31 and AR-Smads had indicated low expression. Reaching their peaks at 14 days after surgery ,TGF-131 and AR-Smads expression was significantly higher than that in normal non-injury control group at day 7,14 ,and 21 postoperatively. [ Conclusion] The animal model of rat bone tendon junction injury, simulates the healing process after BTJ, is a new type and reliable ahimal model.
