抗着丝粒蛋白B抗体在风湿性疾病中的表达及临床意义

摘　　要：目的探讨抗着丝粒蛋白B(CENP-B)抗体在风湿性疾病中的表达及临床意义。方法免疫荧光法和免疫印迹法检测患者血清抗核抗体谱。回顾性分析90例抗CENP-B阳性患者的病种分布和115例确诊为系统性硬化症(SSc)患者的抗核抗体特点及临床资料。结果抗CENP-B抗体阳性见于SSc(33.33%)、原发性胆汁性肝硬化(PBC,24.44%)、系统性红斑狼疮(SLE,16.67%)、干燥综合征(pSS,7.77%)、混合性结缔组织病(MCTD,6.67%)、类风湿关节炎(RA,5.56%)及其他风湿性疾病(5.56%)。115例SSc患者中,70例为弥漫型(60.87%),45例为局限型(39.13%);抗核抗体阳性率为92.17%(106/115),抗SCL-70和抗CENP-B在SSc中的阳性率分别为33.04%(38/115)和26.09%(30/115)。抗SCL-70在弥漫型SSc中的阳性率为54.29%(38/70),抗CENP-B在局限型SSc中的阳性率为66.67%(30/45)。抗SCL-70阳性患者肺间质纤维化发生率高(P<0.05),而抗CENP-B阳性患者肺动脉高压发生率高(P<0.05)。结论抗CENP-B抗体可见于多种风湿性疾病,以SSc患者多见。抗CENP-B和抗SCL-70联合检测有助于SSc的早期诊断。Objective To investigate the expression and significance of anti-centromere protein （CENP）-B antibody in rheumatic diseases. Methods Serum antinuclear antibodies （ANA） were detected by immunofluorescence assay and immunoblotting. Data of 90 patients with anti-CENP-B antibody positive and 115 patients with systemic sclerosis（SSc） were analysed retrospectively. Results Anti-CENP-B antibody was detected in SSc（33.33%）, PBC（24. 44%） ,SLE（16.67%） ,PSS（7.77%）, MCTD（6.67%） ,RA（5.56%） and other rheumatic diseases（5.56%）. Of 115 patients with SSc,there were 70 patients with diffuse type and 45 cases were with limited type, of whom the positive rate of ANA was 92.17% （106/115）, that of SCL-70 was 33.04% （38/115） and that of CENP-B was 26.09 % （30/115）, respectively. The positive rate of SCL-70 in SSc of diffuse type was 54. 29 % （38/ 70） and CENP-B in SSc of limited type was 66.67%（30/45）. The incidence of pulmonary intestitial fibrosis（PIF） was common in the patients with SCL-70（P^0. 05） and that of pulmonary artery hypertention（PAH） occurred frequently in the patients with CENP-B（P〈0. 05）. Conclusion Anti- CENP-B antibody is expressed in varied rheumatic diseases, commonly in SSc. Combined detection of CENP-B and SCL-70 is helpful for early diagnosis of SSc.
