血管性阳痿的药物性阴茎双功能超声研究

摘　　要：目的　评估药物性阴茎双功能超声(PPDU)诊断血管性阳痿的临床价值。方法　受检者57例,对照组35例为心理性阳痿,静脉性阳痿19例,动脉性阳痿3例。用BiosoundAU4彩色超声诊断仪检查注射复方罂粟碱前后的阴茎深动脉和背深静脉的内径和脉冲多普勒的血流频谱。结果　对照组和静脉性阳痿组的PSV高于动脉性阳痿组;静脉性阳痿组的EDV高于对照组和动脉性阳痿组(P<001);静脉性阳痿组的RI均>0,而对照组和动脉性阳痿组的RI≤0;动脉性阳痿的ACC低于对照组和静脉性阳痿组;静脉性阳痿组的VV较对照组和动脉性阳痿组快(P<001)。结论　PPDU是诊断血管性阳痿的一个有效的诊断方法。Objective To assess the value of pharmacopenile duplex Ultrasonography(PPDU) for diagnosis of arteriogenic impotence. Methods Of 57 impotent men with 35 psychogenic impotence,19 venous impotence,3 arteriogenic impotence were examined with Biosound\|AU4 unit.Duplex sonography was performed before and after intracavernosal injection of papaverine.Peak\|systolic velocity(PSV),end\|diastolic velocity(EDV),resistance index(RI) and accelerationin the cavernosal arteries were measured,vein velocity (VV) were detemined for the deep dorsal vein. Results Significant differences in Doppler measurements were found among psychogenic impotence,venous impotence,arteriogenic impotence.PSV in psychogenic impotence group and venous impotence group were higher than that in arteriogenic impotence group.EDV is higher in group of venous impotence than in psychogenic impotence group and arteriogenic impotence groups.RI in group of venous impotence is plus,while that in groups of psychogenic impotence group and arteriogenic impotence group is minus. The group of arteriogenic impotence has a lower ACC than psychogenic impotence group and venous groups.VV in venous impotence group is higher than that in psychogenic impotence group and arteriogenic impotence group. Conclusion PPDU is effcient method for diagnosing vascular impotence. [
